目的应用实时剪切波弹性成像(SWE)技术与甲状腺影像报告与数据系统(TI-RADS)联合,探讨一种更准确评价甲状腺实性结节良恶性的方法。方法对129例患者共147个甲状腺实性结节行常规超声检查,并应用病灶杨氏模量平均值(Emean)对分级进一步调整,分析调整前后诊断效能差异。结果最佳界值为49.51kPa,调整前、后ROC曲线下面积(AUC)分别为0.847、0.946,差异有统计学意义(P=0.009)。结论联合剪切波可更准确评估甲状腺结节的良恶性。
Objective To explore a more accurate method to evaluate benign and malignant thyroid solid nodules by using real-time shear wave elastography (SWE) and thyroid imaging report and data system (TI-RADS). Methods A total of 147 thyroid solid nodules from 129 patients underwent routine ultrasonography. Emean was used to further adjust the grading. The difference in diagnostic efficacy before and after adjustment was analyzed. The best cutoff value was 49.51 kPa. The area under the ROC curve before and after adjustment was 0.847 and 0.946, respectively, with significant difference (P = 0.009). Conclusion Combined shear wave can be more accurate assessment of benign and malignant thyroid nodules.
