目的检测中低位直肠癌组织中转录因子E2F-4的表达,并分析其与临床病理学参数和预后的关系。方法选取临床资料与肿瘤病理标本完整的60例中低位直肠癌患者,所有患者均行直肠癌根治术,另以肠镜活检30例慢性结肠炎的直肠黏膜组织作为对照。免疫组织化学法检测直肠癌组织、癌旁组织及对照直肠黏膜的E2F-4表达,分析其与患者性别、年龄、肿瘤大小、肿瘤分化程度、淋巴结转移、TNM分期和术后转移复发等临床病理参数以及预后的关系。结果直肠癌组织E2F-4表达显著高于癌旁组织和对照直肠黏膜(P<0.05);直肠癌组织E2F-4表达与肿瘤分化程度、TNM分期、淋巴结转移和术后转移复发有关;直肠癌组织E2F-4表达阴性者术后平均无瘤生存时间显著大于阳性者(P<0.05)。结论转录因子E2F-4与直肠癌生物学行为密切相关,在中低位直肠癌的发生发展中可能发挥癌基因的作用。
Objective To detect the expression of transcription factor E2F-4 in low and middle rectal cancer tissues and to analyze its relationship with clinicopathological parameters and prognosis. Methods Totally 60 cases of low and middle rectal cancer patients with clinical data and tumor pathological specimens were selected. All patients underwent rectal cancer radical mastectomy. The other 30 patients with chronic colitis underwent colonoscopy were selected as controls. The expression of E2F-4 in rectal cancer tissues, adjacent non-cancerous tissues and control rectal mucosa was detected by immunohistochemistry. The relationship between E2F-4 expression and the clinicopathological features was analyzed, such as gender, age, tumor size, tumor differentiation, lymph node metastasis, TNM staging and postoperative metastasis The relationship between parameters and prognosis. Results The expression of E2F-4 in rectal cancer tissues was significantly higher than that in paracancerous tissues and control rectal mucosa (P <0.05). The expression of E2F-4 in rectal cancer tissues was correlated with the degree of tumor differentiation, TNM stage, lymph node metastasis and postoperative metastasis and recurrence. The mean time to tumor-free survival of patients with negative E2F-4 expression was significantly higher than that of positive ones (P <0.05). Conclusion The transcription factor E2F-4 is closely related to the biological behavior of rectal cancer and may play an oncogene role in the pathogenesis of low and middle rectal cancer.
