目的:探讨6σ管理方法在门诊处方管理中的应用。方法:将6σ管理方法引入医院门诊处方管理过程之中,按照6σ管理的五个步骤,对门诊处方差错进行“诊断”与干预管理。分析6σ管理方法使用前后药疗差错(如处方差错、调剂差错及给药差错三个方面)、药品合格情况以及改进后处方缺陷情况等。结果:6σ管理方法使用后,①处方差错、调剂差错及给药差错均明显要低,且差异具有显著的统计学意义(P<0.01);②门诊用药合格率达到100%,明显高于该方法使用前的用药合格率,二者具有显著的统计学差异(P<0.01);③护士满意率为100%,明显高于该方法使用前,二者具有显著的统计学差异(P<0.01);④6σ管理方法实施前处方缺陷率为14.28%,改善后处方缺陷率为6.25%,二者呈现出显著的统计学差异(P<0.01)。结论:将6σ管理方法应用于医院门诊处方管理过程之中,能够明显降低处方药疗差错率与处方缺陷率、大大提高门诊用药合格率。
Objective: To explore the application of 6σ management in outpatient prescription management. Methods: The 6σ management method was introduced into the hospital outpatient prescription management process. According to the five steps of 6σ management, the “diagnosis” and intervention management of outpatient prescription errors were carried out. Analysis 6σ management methods before and after the use of medical errors (such as prescription errors, dispensing errors and medication errors in three areas), drug eligibility and improved prescription defects and so on. Results: After the 6 sigma management method was used, ① prescription errors, medication errors and medication errors were significantly lower, and the difference was statistically significant (P <0.01); ② outpatient medication pass rate of 100%, significantly higher than the (P <0.01). The satisfaction rate of nurses was 100%, which was significantly higher than that before the method was used (P <0.01), and there was a significant difference between the two methods ); ④6σ management method before the implementation of the prescription defect rate was 14.28%, after the improvement of the prescription defect rate was 6.25%, the two showed a significant statistical difference (P <0.01). Conclusion: The application of 6σ management method in hospital outpatient prescription management can significantly reduce the prescription medical error rate and the prescription defect rate, and greatly improve the outpatient drug acceptance rate.
