目的探讨应用多排螺旋CT血管成像(multi-slice CT angiography,MSCTA)研究支气管动脉(bronchial artery,BA)的解剖特征及其临床价值。方法 120例咳血患者行MSCTA检查,男93例,女27例,年龄16~85岁,平均年龄52.4岁。对其数据进行容积再现(volume rendering,VR)、最大密度投影(maximum intensity projection,MIP)、多平面重组(multi-planar reformation,MPR)及曲面重建(curved planar reformation,CPR)等三维后处理,获得支气管动脉的起源、开口位置、分支类型等解剖信息。结果对120例咳血患者的支气管动脉进行统计,发现起源正常支气管动脉271支(271/292,92.8%),异位起源支气管动脉21支(21/292,7.2%)。分支类型R1L1型最常见占34.3%,其次较为常见的类型为R2L119.2%、R1L015.2%、R1L210.1%。右侧支气管动脉起源于右前壁(100/105,95.2%);左侧支气管动脉起源于左前壁(67/78,85.9%);共干支气管动脉起源于前壁(28/36,77.8%)。结论 MSCTA可清晰显示支气管动脉,有效评价支气管动脉的三维影像解剖特征,对辅助支气管动脉栓塞介入手术插管具有肯定的临床价值,并能同时对肺部疾病进行评价。
Objective To investigate the anatomic characteristics of bronchial artery (BA) and its clinical value using multi-slice CT angiography (MSCTA). Methods 120 cases of hemoptysis patients underwent MSCTA examination, 93 males and 27 females, aged 16 to 85 years with an average age of 52.4 years. The data were subjected to three-dimensional post-processing such as volume rendering (VR), maximum intensity projection (MIP), multi-planar reformation (MPR) and curved planar reformation (CPR) Obtain the origin of bronchial artery, the location of the opening, the type of branch and other anatomical information. Results The bronchial arteries of 120 patients with hemoptysis were counted and found that there were 271 (291/292, 92.8%) normal origin bronchial arteries and 21 (29/92, 72%) ectopic origin bronchial arteries. The most common type of branch type R1L1 accounted for 34.3%, followed by the more common types of R2L119.2%, R1L015.2%, R1L210.1%. The right bronchial artery originated in the right anterior wall (100/105, 95.2%); the left bronchial artery originated in the left anterior wall (67/78, 85.9%); the common dry bronchial artery originated in the anterior wall (28/36, 77.8 %). Conclusion MSCTA can clearly show the bronchial artery, effectively evaluate the three-dimensional anatomical characteristics of the bronchial artery, confirm the clinical value of interventional bronchial artery embolization and can also evaluate the pulmonary disease.
