目的探讨人乳头瘤病毒感染与不同部位鲍温病(Bowen)病发病的关系并检测肿瘤错配修复基因hMLH1,hMSH2蛋白在Bowen病中的表达情况。方法 20例Bowen病组织来源于2004-2011年皮肤科病理证实标本,其中肛周和外生殖器共3例,其他部位共17例,另取10例正常皮肤为对照。采用人乳头瘤病毒核酸扩增分型方法对Bowen病皮损进行基因分型检测。免疫组化法检测Bowen病组织及10例正常皮肤组织中hMLH1,hMSH2蛋白的表达。结果 20例Bowen病患者中肛周和外生殖器、其他部位共检出6例HPV阳性(30.00%),为HPV16,HPV18,HPV33型。Bowen病组织中hMLH1,hMSH2蛋白阳性表达较正常皮肤组织降低,差异有统计学意义(P<0.05);Bowen病高危型HPV感染组中hMLH1,hMSH2蛋白阳性表达低于非感染组(P<0.05)。结论 Bowen病的发生可能与HPV感染有关,Bowen病患者皮损中hMLH1,hMSH2蛋白低表达也许与其进展为肿瘤有关联。
Objective To investigate the relationship between human papillomavirus (HPV) infection and the pathogenesis of Bowen’s disease at different sites and to detect the expression of tumor mismatch repair genes hMLH1 and hMSH2 in Bowen’s disease. Methods Twenty cases of Bowen’s disease were from dermatologically confirmed specimens from 2004 to 2011, including 3 cases of perianal and external genitalia, 17 cases of other parts and 10 cases of normal skin as control. The genotypes of Bowen’s lesions were detected by nucleic acid amplification of human papillomavirus (HPV). Immunohistochemistry was used to detect the expression of hMLH1 and hMSH2 protein in Bowen’s disease tissues and 10 normal skin tissues. Results In 20 cases of Bowen’s disease, 6 cases of HPV positive (30.00%) were detected in other parts of the perianal and external genitalia, which were HPV16, HPV18 and HPV33. The positive expression of hMLH1 and hMSH2 protein in Bowen’s disease tissues was lower than that in normal skin tissues (P <0.05). The positive rates of hMLH1 and hMSH2 protein expression in Bowen’s disease high-risk HPV infection group were lower than those in non-infected group (P <0.05) ). Conclusions The occurrence of Bowen’s disease may be related to HPV infection. The low expression of hMLH1 and hMSH2 in the lesion of Bowen’s disease may be related to its progression to tumor.
