目的:旨在分析龙泉市人民医院铜绿假单胞菌的产酶情况,单独、联合药敏试验结果对临床用药的指导意义,为临床对抗菌药物的选择提供合理而准确的科学依据。方法:收集136株铜绿假单胞菌做为研究对象,使用世界卫生组织细菌耐药性监测中心推荐的WHONET5.5和EXCEL软件进行数据回顾性分析。结果:产酶铜绿假单胞菌与非产酶铜绿假单胞菌的药物敏感性差异有统计学意义(P<0.05),并且在多重耐药铜绿假单胞菌和泛耐药铜绿假单胞菌情况下,头孢哌酮/舒巴坦与左氧氟沙星,多粘菌素B与利福平的联合用药在体外抗菌活性以相加和协同作用为主。结论:铜绿假单胞菌在临床分离率较高,耐药情况相对严重,耐药机制相对复杂,微生物实验室应及时监测铜绿假单胞菌药物敏感性的变化趋势。在多重耐药与泛耐药情况下,微生物实验室在条件允许情况下,应开展联合药敏试验,为临床在抗菌药物选择性困难的时候提供科学的实验依据。
OBJECTIVE: To analyze the enzyme production of Pseudomonas aeruginosa in Longquan People’s Hospital. The results of single and combined drug susceptibility testing are of guiding significance to clinical medication and provide a reasonable and accurate scientific basis for the selection of antibacterial drugs. Methods: 136 strains of Pseudomonas aeruginosa were collected and analyzed retrospectively using WHONET5.5 and EXCEL software recommended by the WHO Center for Bacteriological Drug Resistance Surveillance. Results: The drug sensitivity of Pseudomonas aeruginosa producing and non-producing Pseudomonas aeruginosa was statistically significant (P <0.05), and in multi-resistant Pseudomonas aeruginosa and pan-resistant Pseudomonas aeruginosa single The combination of cefoperazone / sulbactam with levofloxacin, polymyxin B and rifampicin combined antibacterial activity in vitro with additive and synergistic effects. Conclusion: Pseudomonas aeruginosa has a high clinical isolation rate, relatively serious drug resistance and a relatively complicated drug resistance mechanism. Microbiological laboratory should monitor the trend of drug sensitivity of Pseudomonas aeruginosa in a timely manner. In the case of multiple drug resistance and pan-drug resistance, microbiological laboratories should carry out joint susceptibility testing under conditions permitting to provide a scientific experimental basis for the clinical selection of antibacterial drugs.
