目的:探究高压氧联合旋刀片式股骨近端髓内钉(proximal femoral nail antirotation, PFNA)内固定治疗股骨粗隆间骨折的临床疗效,并分析其对患者血液流变学指标、骨折愈合情况及关节功能的影响。方法:选取解放军第九六○医院淄博院区2018年6月至2019年6月收治的92例股骨粗隆间骨折患者作为研究对象。将92例股骨粗隆间骨折患者按随机数字表法分为对照组(n n=46)和观察组(n n=46)。2组患者均采取PFNA内固定术治疗,术后采取常规抗感染等治疗。观察组患者则在此基础上联合高压氧辅助治疗。观察2组患者临床疗效、血液流变学指标、骨密度及骨愈合状况,同时评估患者术后髋关节功能恢复情况。n 结果:治疗后,观察组的血液黏度、红细胞聚集指数及红细胞沉积均显著低于对照组,差异有统计学意义(n P<0.05);而纤维蛋白原水平明显高于对照组,差异有统计学意义(n P<0.05)。治疗3个月及6个月后,观察组骨密度及骨折愈合情况分别为-2.15±0.13、73.91%及-1.96±0.10、93.48%,均优于对照组的-2.30±0.16、54.35%及-2.11±0.14、82.61%,差异有统计学意义(n P<0.05);观察组临床有效率为91.30%,显著高于对照组76.09%,差异有统计学意义(n P<0.05);且术后髋关节功能均优于对照组,差异有统计学意义(n P<0.05)。n 结论:高压氧联合PFNA内固定治疗股骨粗隆间骨折的临床疗效显著,术后血液流变学指标及骨密度均有显著改善,能有效提高骨折愈合率,促进髋关节功能恢复。“,”Objective:To explore the clinical effect of hyperbaric oxygen combined with proximal femoral nail anti-rotation (PFNA) internal fixation in patients with femoral intertrochanteric fracture, and to analyze its effect on hemorheology indexes, fracture healing, and joint function.Methods:A total of 92 patients with femoral intertrochanteric fracture admitted to Zibo Branch of CPLA No.960 Hospital from June 2018 to June 2019 were selected as the research subjects and randomly divided into control group (n n=46) and observation group (n n=46). Patients in both groups were treated with PFNA internal fixation and conventional anti-infection treatment, while the patients in the observation group were additionally treated with hyperbaric oxygen. The clinical efficacy, hemorheological indexes, bone density, and bone healing status of the two groups were observed. And the patients’ postoperative function of the hip joint was also assessed.n Results:After the treatment, the blood viscosity, erythrocyte aggregation index, and erythrocyte sedimentation rate in the observation group were all lower than those in the control group, and all the differences were statistically significant (n P<0.05). And the fibrinogen level was significantly higher than that in the control group (n P<0.05). Bone mineral density and fracture healing status in the observation group were -2.15±0.13, 73.91% and -1.96±0.10, 93.48% after 3 and 6 months of treatment respectively, which were better than those in the control group -2.30±0.16, 54.35% and -2.11±0.14, 82.61%(n P<0.05). The clinical effective rate of the observation group (91.30%) was significantly higher than that in the control group (76.09%) with statistically significant difference (n P<0.05). The postoperative function of hip joint was also significantly improved than that of the control group (n P<0.05).n Conclusion:The clinical efficacy of the hyperbaric oxygen combined with PFNA internal fixation in the treatment of femoral intertrochanteric fracture was significant. It can improve postoperative hemorheology indexes, increase bone density and fracture healing rate, and improve the function of hip joint. It is worthy of clinical promotion and application.
