自发性蛛网膜下腔出血(以下简称SAH)是指脑底面或脑表面的血管发生病变、破裂,血液流入蛛网膜下腔而言。发病率为0.5~2%,占脑血管病的5~12%。主要病因是脑动脉瘤和脑血管畸形,两者比例大致相等。本病起病急骤,临床上突然发生剧烈头痛、恶心、呕吐,可有一过性意识障碍及脑膜刺激征,脑脊液呈血性,诊断较易。但部分病人表现错综复杂,且有明显的定位体征,给诊断和治疗增加困难。本文从以下若干问题加以讨论,旨在提高对本病的认识。
Spontaneous subarachnoid hemorrhage (hereinafter referred to as SAH) refers to the bottom of the brain or cerebrovascular surface lesions, rupture, the blood flow into the subarachnoid space. The incidence rate of 0.5 to 2%, accounting for 5 to 12% of cerebrovascular disease. The main cause is cerebral aneurysm and cerebrovascular malformations, the proportion of both roughly equal. The sudden onset of this disease, a sudden clinical severe headache, nausea, vomiting, may have a transient disturbance of consciousness and meningeal irritation, cerebrospinal fluid was bloody, easier to diagnose. However, some patients showed intricate symptoms and obvious signs of positioning, which made it difficult to diagnose and treat. This article discusses a number of issues below to raise awareness of the disease.
