患者女,84岁.双眼视物不清10余年,近来左眼视力下降明显.查视力:右眼0.12,左眼0.5,双眼矫正视力不提高.双眼前节未见明显异常,双眼晶体混浊,以右眼为重,左眼玻璃体腔内可见絮状混浊物,右眼眼底检查:视盘边界清楚,视网膜血管走形大致正常,黄斑区见一约2DD大小的黄色圆形病灶.左眼眼底检查:视盘边界清楚,视网膜血管走形正常,中心凹反光不见.右眼红外照相示黄斑区可见一约2PD大小的弱荧光;荧光素

患者女,84岁.双眼视物不清10余年,近来左眼视力下降明显.查视力:右眼0.12,左眼0.5,双眼矫正视力不提高.双眼前节未见明显异常,双眼晶体混浊,以右眼为重,左眼玻璃体腔内可见絮状混浊物,右眼眼底检查:视盘边界清楚,视网膜血管走形大致正常,黄斑区见一约2DD大小的黄色圆形病灶.左眼眼底检查:视盘边界清楚,视网膜血管走形正常,中心凹反光不见.右眼红外照相示黄斑区可见一约2PD大小的弱荧光;荧光素血管造影示黄斑区"轮辐样"强荧光渗漏;吲哚青绿血管造影黄斑区见一大的新生血管主干,此主干向外伸出大量的"枝条样"分支;24分18秒时荧光素眼底血管造影黄斑区强荧光渗漏更加明显,吲哚青绿血管造影黄斑区见一略低荧光区.左眼红外照相示黄斑区见片状不规则低红外反光区;488nm自发荧光及790红外自发荧光未见明显异常;荧光素未见明显异常;吲哚青绿血管造影1分23秒时显示黄斑区见一竖条形强荧光,其两旁可见"葡萄样"弱荧光.23分42秒时眼底荧光素血管造影未见明显改变,吲哚青绿血管造影显示黄斑区"葡萄样"弱荧光增强明显.右眼OCT显示黄斑区视网膜神经上皮局部浅脱离,色素上皮较广泛脱离,脱离的神经上皮及色素上皮之间可见中等反光物质,神经上皮内见液性暗腔.左眼OCT显示黄斑区视网膜色素上皮呈丘状脱离,部分融合.
