Symptomatic intracerebral hemorrhage (ICH) is the most dangerous adverse effect of tissue plasminogen activator (tPA), the only FDA-approved drug, for treating acute ischemic stroke patients.Currently, neither reliable indicators for excluding stroke patients at high-risk for ICH nor effective strategies for reducing ICH are available.Accumulating evidence suggests that disruption of the blood brain barrier (BBB) can be early enough to be within the 3-hr thrombolytic time window, and this hyperacute BBB damage shows great potential to predict ICH following thrombolytic therapy for ischemic stroke.
