  Biologic markers that predict presence or subsequent development of invasive disease in patents diagnosed with ductal carcinoma in-situ (DCIS) are needed for more personalized targeted treatment.Subclassification of breast cancer based on cDNA expression profile which could be reflected by immunohistochemical detection for ER, PR and HER-2/neu,have recently been shown to have prognostic significances in invasive disease.The implication of the similar phenotypic patterns in ductal carcinoma in situ (DCIS) remains uncertain.In this presentation, data from a study based on 104 patients who had DCIS on clinical and imaging study initially will be presented.The incidence of early invasive disease in this group of patients was analyzed in association with the immunphenotypic classification and various histopathologic features of the DCIS.The prognostic and therapeutic significance of HER2 overexpression and ER/PR/HER2 phenotype in DCIS will be discussed.A HER2 vaccine therapy which is currently in clinical trial phase in the Hospital of the University of Pennsylvania will be briefly reviewed and its potential impact on early invasive disease will be discussed.
