Background: A recent meta-analysis focussing on 5-HTTLPR (Li and He, 2007) suggests differential effects mediated by ethnicity. Methods: We examined the association between suicide and the functional serotonin transporter polymorphism (5-HTTLPR) as well as the role of economic indices, national alcohol intake and national happiness scores.Data on allelic frequency from 126.697 healthy screened or general population individuals from 40 different countries were analysed with multiple regressions. Results: The results emphasize the role of alcohol intake and mood as risk factors in suicidal behaviour. There is no overall effect of 5-HTTLPR on suicide, but the results reveal a differential effect according to ethnicity. The short (S) allele acts as a protective factor in Caucasian populations and as a risk factor in non-Caucasian populations. Conclusion: The apparent interaction may be due to (1) a cline, (2) adjacent polymorphisms, (3) genetic cascades, or (4) gene-environment interaction.Cross-country research proves to be a vital tool in the understanding and identification of risk factors for suicidal behaviour.
