Aim: The LD flap is one of the most commonly used techniques for partial breast reconstruction (PBR), but has the disadvantages of donor-site morbidity and deformity.Laparoscopically harvested omental flap (OF) may compensate for these disadvantages.We present our unique oncoplastic technique of PBR after breast-conserving surgery (BCS) using the OF.Methods: PBR using the OF was applied immediate after BCS to 106 patients.A wide excision (usually more than 30% of breast tissue) was carried out, then the OF was laparoscopically harvested.The OF was usually extracted via a small subcutaneous tunnel created from the xyphoid process, and filled the partial mastectomy defect.The patients were followed-up for complications and cosmetic results.Results: Median operative time was 4.0 hours.Complication rate is 10.5 %, and all of them were minor and treated conservatively.Cosmetic results were almost satisfactory, with a soft breast that was natural in appearance.In 13 patients, minor deformity or asymmetry of the reconstructed breast had occurred because of postoperative infectious complications or volume lack.Donor-site scars were minimal like those of laparoscopic cholecystectomy.Conclusion: The OF is a safe procedure with minimal donor-site morbidities and deformities, and an additional option for PBR after BCS.
