目的研究金矿从业人员的健康指标异常情况,分析其脂肪肝和肝功能异常的主要影响因素。方法收集河南省三门峡市某金矿从业人员的健康体检资料,其中从事粉尘暴露作业人员388名,汞暴露作业人员102名,氰化物暴露作业人员26名,采用问诊及仪器测量相结合的方式,分析从业者高血压、脂肪肝、肝功能异常、血脂和胆红素异常情况,采用Logistic回归分析了脂肪肝和肝功能异常的主要影响因素。结果该职业人群中,脂肪肝和肝功能异常的检出率分别为13.2%和8.5%,高血压的检出率为16.9%,血脂异常和胆红素异常的检出率分别为17.6%和8.3%;脂肪肝和血脂异常与肝功能异常相关OR分别为2.81(95%CI:1.07-7.37)和3.22(95%CI:1.77-5.86);与粉尘暴露者相比,汞暴露者肝功能异常的风险相对较低(OR=0.30,95%CI:0.10-0.87);男性较女性发生脂肪肝的风险相对增加(OR=3.37,95%CI:1.28-8.85);与粉尘暴露者相比,汞和氰化物暴露患脂肪肝风险相对较高,其OR分别为9.80(95%CI:4.50-21.32)和14.80(95%CI:5.50-21.32)。结论高血压、脂肪肝、肝功能异常是金矿从业者较为常见的健康异常改变;血脂异常和脂肪肝会增加金矿从业者肝功能异常的发生风险;与女性从业人员相比,男性发生脂肪肝的风险相对增加;汞和氰化物暴露会增加脂肪肝的发生风险。
Objective To study the abnormality of health indicators of gold miners and analyze the main influencing factors of fatty liver and abnormal liver function. Methods The health checkup data of a gold mine employees in Sanmenxia City of Henan Province were collected. Among them 388 dust exposure workers, 102 mercury exposure workers and 26 cyanide exposure workers were employed.
