目的对微造瘘经皮肾镜取石术治疗输尿管上段结石并重度肾积水患者的临床效果进行分析探讨。方法对2010年1月~2013年6月在本院采用造瘘经皮肾镜取石术治疗输尿管上段结石并重度肾积水的150例患者的临床资料进行回顾性分析。结果 60例患者建立通道后寻找肾盂输尿管连接部时间(18±7)min,平均手术时间(56.5±15)min,肾造瘘管平均置留时间为(4.5±0.5)d,平均住院时间为(7.0±0.5)d,清石率90%,有6例患者有小于0.7vm的残留结石存在,并进行了体外冲击结石治疗,取得良好的效果。且在治疗过程中均无大出血、感染等严重的并发症出现。结论微造瘘经皮肾镜取石术治疗输尿管上段结石并重度肾积水有很好的临床疗效,有一定的临床应用效果。
Objective To analyze the clinical effect of percutaneous nephrolithotomy on patients with upper ureteral calculi and severe hydronephrosis. Methods The clinical data of 150 patients with upper ureteral calculi and severe hydronephrosis treated with ostomy by nephrolithotomy in our hospital from January 2010 to June 2013 were analyzed retrospectively. Results The time of finding the connection of ureteropelvic junction in 60 patients was 18 ± 7 min, the average operation time was 56.5 ± 15 min, the mean stay time of renal fistula was 4.5 ± 0.5 d, and the average length of stay was ( 7.0 ± 0.5) d. The rate of clear stone was 90%. There were 6 residual stones less than 0.7vm in 6 patients, and the external impact stones were treated with good results. And no major bleeding in the course of treatment, infection and other serious complications appear. Conclusion Micro-percutaneous nephrolithotomy lithotripsy for the treatment of upper ureteral calculi and severe hydronephrosis has a good clinical efficacy, a certain clinical application.
