本文同步测定了18例Ⅱ型呼吸衰竭患者CSF和动脉血的血气分析,并与15例健康正常人作对照研究。结果发现:对照组CSF pH非常显著低于血pH(P<0.01),P_(CSF)CO_2高于PaCO_2而P_(CSF)O_2低于PaO_2(P<0.01),CSF的BE、AB和BB均比动脉血显著低(P<0.01);呼衰组P_(CSF)CO_2和PaCO_2相差不明显(P>0.05),P_(CSF)O_2明显高于PaO_2,CSF BE、AB和BB亦明显低于动脉血水平;呼衰时P_(CSF)CO_2显著大于正常P_(CSF)CO_2,由于前者HCO_3代偿性增多,其pH却明显高于后者(P<0.01),而呼衰时P_(CSF)O_2大大地高于正常P_(CSF)O_2。正常对照组CSF pH与P_(CSF)CO_2存在明显的负相关关系(n=15,r=-0.792 P<0.01),但与其余四项指标均无明显关系(P>0.05);呼衰组CSF pH与P_(CSF)CO_2以及其他指标之间全无相关关系(P>0.05)。
In this paper, blood gas analysis of CSF and arterial blood in 18 patients with respiratory failure Ⅱ was simultaneously measured and compared with 15 healthy controls. The results showed that the CSF pH of the control group was significantly lower than that of the blood pH (P <0.01). The P_ (CSF) CO_2 was higher than PaCO_2 and the P_ (CSF) O_2 was lower than PaO_2 (P <0.01) The difference of P_ (CSF) CO_2 and PaCO_2 was not significant (P> 0.05), P_ (CSF) O_2 was significantly higher than that of PaO_2, CSF BE, AB and BB were also significantly lower than those of arterial blood (P <0.01). However, P_ (CSF) CO_2 was significantly higher than that of normal P_ (CSF) CO_2 at the time of respiratory failure, and the pH was significantly higher than that of the latter due to the compensatory increase of HCO_3 in the former (P <0.01) ) O 2 is significantly higher than normal P_ (CSF) O_2. There was a significant negative correlation between CSF pH and P_ (CSF) CO_2 in normal control group (n = 15, r = -0.792 P <0.01), but no significant relationship with the other four indexes (P> 0.05) There was no correlation between CSF pH, P_ (CSF) CO_2 and other parameters (P> 0.05).
