种植体支抗在安氏Ⅱ1类错(牙合)非常规拔牙矫治中的应用研究

目的:分析探讨微螺钉种植体支抗技术在安氏Ⅱ类1分类错牙合非常规拔牙矫治中的应用效果。方法:选择安氏Ⅱ类1分类错牙合患者32例,年龄15~40岁,所有患者经头影测量和模型分析都是上颌需要最大支抗的病例,其中上颌拔除左右两个第一双尖牙共计17例,上颌由于第二双尖牙不同程度龋坏而拔除左右两个第二双尖牙或一侧拔除第一双尖牙而另一侧拔除第二双尖牙共计15例。合理设计种植体植入部位,在左右第一磨牙近中牙槽间隔植入微螺钉种植体。矫治前后拍摄头颅侧位片,比较矫治前后第一磨牙的前移量、上前牙的内收量。结果:所有病例拔牙间隙都完全关闭,上前牙内收效果明显,1/NA角从33.7°减小到22.5°,平均减小11.2°(P<0.01);1-NA距从10.3mm减小到5.9mm,平均减小4.4mm(P<0.01);6-参考线(经翼上颌裂点与FH垂线)距离从22.8mm增加至23.7mm,平均增加0.9mm(P>0.05),说明上颌第一磨牙前移不明显;SNA、SNB、1/NB的变化均无统计学意义。结论:微螺钉种植体可以为那些非常规拔牙患者需要最大支抗的病例提供稳定而可靠的支抗。 Objective: To analyze the application effect of micro-screw implant support technique in Class Ⅱ division 1 malocclusion unconventional tooth extraction. Methods: Thirty-two patients with Class Ⅱ division 1 malocclusion were selected, aged from 15 to 40 years old. All patients were measured by cephalometric analysis and model analysis. The distance between the 6-reference line (the maxilla and FH vertical line) increased from 22.8mm to 23.7mm with an average increase of 0.9mm (P> 0.05) The maxillary first molars did not move forward obviously. The changes of SNA, SNB and 1 / NB were not statistically significant. Conclusions: Micro-screw implants provide stable and reliable support for those patients who require maximal support in unconventional patients.
