目的:探讨清道夫受体B族Ⅰ型基因(SR-BI)外显子8单核苷酸多态性对中国汉族人群冠心病发病易感性及血脂水平的影响。方法:应用聚合酶链反应-限制性片段长度多态性分析方法 ,测定370例冠心病患者和143例正常对照者SR-BI基因外显子8的HaeⅢ酶切基因型,分析其与冠心病、冠脉造影结果、血脂水平的关系,采用Logistic回归了解基因型与冠心病危险因素间的相互作用。结果:SR-BI基因外显子8等位基因C和T频率在冠心病组为0.809、0.191,在对照组为0.790、0.210。冠心病组与对照组比较,SR-BI外显子8 HaeⅢ酶切基因型和等位基因分布频率差异无统计学意义(P>0.05)。对照组中SR-BI基因外显子8CT+TT基因型亚组低密度脂蛋白胆固醇(LDL-C)、脂蛋白a[Lp(a)]水平显著高于同组CC基因型亚组(P<0.05)。对照组中CT+TT基因型亚组女性LDL-C、Lp(a)水平显著高于同组CC基因型亚组(P<0.05)。高血压史、糖尿病史、吸烟史、总胆固醇(TC)、LDL-C、Lp(a)为冠心病的独立危险因素,OR值分别为2.007、2.395、3.650、1.405、1.405、1.377、1.615;高密度脂蛋白胆固醇(HDL-C)、载脂蛋白AI(Apo-AI)为冠心病的保护因素,OR值分别为0.256、0.157。结论:SR-BI基因外显子8多态性可能与中国汉族人群冠心病发病易感性及冠心病病变严重程度无关,但SR-BI基因外显子8T等位基因可引起女性LDL-C和Lp(a)水平升高。
Objective: To investigate the effect of single nucleotide polymorphism of scavenger receptor B-type Ⅰ gene (SR-BI) on susceptibility to coronary heart disease and blood lipid levels in Chinese Han population. Methods: Polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) was used to detect the Hae Ⅲ genotype of SR-BI exon 8 in 370 CHD patients and 143 healthy controls. , Coronary angiography results, blood lipid levels, the use of Logistic regression to understand the genotype and coronary heart disease risk factors between the interaction. Results: The frequencies of C and T alleles of exon 8 of SR-BI gene in CHD group were 0.809,0.191 and 0.790,0.210 in control group. Compared with the control group, there was no significant difference in genotype distribution and allele frequencies of SR-BI exon 8 Hae Ⅲ between CHD group and control group (P> 0.05). The level of LDL-C and Lp (a) in subgroup of exon 8CT + TT genotype of SR-BI gene in control group was significantly higher than that in subgroup of CC genotype (P <0.05). The levels of LDL-C and Lp (a) in CT + TT genotype subgroup of control group were significantly higher than those in CC genotype subgroup (P <0.05). The history of hypertension, history of diabetes, smoking history, total cholesterol (TC), LDL-C and Lp (a) were independent risk factors for coronary heart disease, with OR values of 2.007, 2.395, 3.650, 1.405, 1.405, 1.377 and 1.615, High-density lipoprotein cholesterol (HDL-C) and apolipoprotein AI (Apo-AI) were the protective factors of coronary heart disease with ORs of 0.256 and 0.167, respectively. Conclusion: The exon 8 polymorphism of SR-BI gene may be related to the susceptibility to coronary heart disease and the severity of coronary heart disease in Chinese Han population. But the exon 8T allele of SR-BI gene may cause female LDL-C and Lp (a) level increased.
