目的:探讨丙泊酚在小型猪心肺复苏模型中对血清S100β蛋白浓度及其表达的影响。方法:小型猪12只,采用电击致颤制作心跳骤停-心肺复苏模型,采用随机数字表法分为丙泊酚组和对照组(10%脂肪乳剂),心跳骤停4 min开始复苏,自主循环恢复(ROSC)后分别给予丙泊酚1 mg/kg,随后按照丙泊酚10 mg/(kg·h)持续静脉泵注6 h,对照组给予同等量的10%脂肪乳剂,观察6 h拔除气管插管,24 h后静脉注射氯化钾处死,取大脑皮层,采用双抗夹心酶联免疫分析法(ELISA)测定两组基线、RSOC即刻(0 min)、15 min、1、2、4、6、24 h血清S100β蛋白浓度,采用免疫组织化学法测定大脑皮层S100β蛋白的表达,统计小型猪24 h存活率。两组间及组内不同时间点均采用n t检验。n 结果:S100β蛋白水平在6 h上升并达峰值,丙泊酚组血清S100β浓度[(461.58±16.72) ng/L]较对照组[(560.82±30.857) ng/L]上升较少,两组差异有统计学意义(n t=-7.341,n P<0.05),24 h丙泊酚组血清S100β浓度[(383.82±29.586) ng/L]较对照组[(463.97±20.463) ng/L]上升低,两组差异有统计学意义(n t=-6.553,n P<0.05),大脑皮层S100β蛋白表达阳性细胞丙泊酚组[(7.2±0.5)个/HP]与对照组[(10.8±0.8)个/HP,n t=7.707,n P<0.05],差异有统计学意义。n 结论:心肺复苏后静脉输注1 mg/kg丙泊酚,后持续6 h静脉输注丙泊酚10 mg/(kg·h),能降低血清中S100β蛋白的浓度及S100β蛋白在大脑皮层的表达,但对于24 h存活率并无明显影响。“,”Objective:To investigate the effect of propofol on the concentration and expression of serum S100β protein in a swine model after cardiopulmonary resuscitation.Methods:The model of cardiac arrest (CA) and cardiopulmonary resuscitation (CPR) was conducted in swines using electric shock induced ventricular fibrillation. 12 swines were divided in to propofol group and control group (10% Intralipid) randomly. CPR was performed after 4 minutes of electric shock induced cardiac arrest, after 15min of restoration of spontaneous circulation (ROSC), propofol group and control group were continuously intravenously pump-infused with propofol 1mg/kg and Intralipid 1 mg/kg for 6h respectively. Removed endotracheal tube after 6h observation, swines were sacrificed with intravenous injection of potassium chloride after 24 h, subsequently, the cerebral cortex was harvested for following experiments. The concentration of serum S100β protein was determined using a enzyme-linked immunosorbent assay (ELISA) kit at different time points, including RSOC immediately (0 min), 15 min, 1 h, 2 h, 4 h, 6 h and 24 h. In addition, the S100β protein expression level was qualified by immunohistochemistry. The measurement in this study was expressed as mean±standard deviation (n ±n s), n T-test was used for data analysis between 2 groups, n P values<0.05 were considered statistically significant.n Results:The concentration of S100β in both group peaked at 6h, at the both time points of 6 h and 24 h, the concentrations of S100β in propofol group [(461.58±16.72) pg/ml and (383.82±29.586) pg/ml] were significantly (n t=-7.341, n P<0.05) lower than that in control group [(560.82±30.857) pg/ml and (463.97±20.463) pg/ml,n t=-6.553, n P<0.05] respectively. The immunohistochemistry results demonstrated that the expression level of S100 β protein was significantly decreased in propofol group [(7.2±0.5)/HP], compared with control group [(10.8±0.8)/HP,n t=7.707, n P<0.05].n Conclusion:Propofol could reduce the concentration of serum S100β protein and the expression level of S100 β protein in cerebral cortex, however had no significantly effect on 24-hour survival rate.
