目的探讨单纯性肥胖对青春期前男童性发育的影响。方法选择6~11岁处于青春发育前期(即Tanner分期Ⅰ期)的单纯性肥胖男童(单纯肥胖组)56例,采用化学发光法检测其血清睾酮(T)、雌二醇(E2)、黄体生成素(LH)、泌乳素(PRL)水平,游标卡尼测量其睾丸平均容积(MTV),同时检测32例健康男童(健康对照组)各项指标并进行比较。结果单纯肥胖组男童血清T及LH水平显著低于健康对照组(Pa<0.01),MTV亦低于健康对照组(P<0.05),轻、中、重单纯肥胖组3组男童T及LH比较也有显著差异(Pa<0.05);单纯肥胖组男童血清E2及PRL水平均明显高于健康对照组(Pa<0.01),肥胖男童轻、中、重3组E2及PRL比较均有显著性差异(Pa<0.05);肥胖组E2与BMI呈正相关(r=0.6659P<0.05),T水平与BMI呈负相关(r=-0.7158P<0.05);肥胖男童轻、中、重3组睾丸MTV比较差异无统计学意义(Pa>0.05)。结论单纯性肥胖可导致青春期前男童性发育不良或性发育迟缓。
Objective To investigate the effect of simple obesity on the sexual development of pre-adolescent boys. Methods 56 cases of simple obesity boys (simple obesity group) aged 6 to 11 years old in pre-puberty (ie Tanner stage Ⅰ) were selected and their serum testosterone (T), estradiol (E2) Luteinizing hormone (LH) and prolactin (PRL) were measured. The testicular mean volume (MTV) was measured by vernier calythrin. Meanwhile, 32 healthy boys (healthy control group) were also detected and compared. Results The levels of serum T and LH in obesity group were significantly lower than those in healthy control group (P <0.01), and MTV was also lower in healthy obese group (P <0.05) (P <0.05). The levels of serum E2 and PRL in obesity group were significantly higher than those in healthy control group (Pa <0.01). The levels of E2 and PRL in obese boys were significantly lower than those in healthy group (R = -0.7158P <0.05). There was a significant positive correlation between E2 and BMI in obese group (r = 0.6659P <0.05), while the level of T was negatively correlated with BMI There was no significant difference in testicular MTV between the three groups (Pa> 0.05). Conclusions Simple obesity can lead to prematurity boys with sexual dysplasia or sexual retardation.
