立体定向血肿抽吸术对大鼠脑出血后NSE表达的影响

目的在Ⅳ型胶原酶诱导的实验性脑出血模型的基础上,实施立体定向血肿抽吸术并研究抽吸术后血肿周围血浆神经元特异性烯醇化酶(NSE)的变化及意义。方法成年健康Wistar雄性大鼠135只,随机分为假手术组、脑出血组、抽吸治疗组。将Ⅳ型胶原酶注入大鼠尾状核建立大鼠脑出血模型,在形成脑出血10 h后,向血肿腔内注入含有3 000 U尿激酶的溶液5μl来溶解血凝块,2 h后进行立体定向血肿抽吸术干预治疗。应用Bederson四级评分法评价大鼠神经功能缺损程度,免疫组化法及Western blot分别检测NSE蛋白的表达。结果大鼠脑出血后,各个时间点脑出血组、抽吸治疗组血肿周围脑组织NSE蛋白表达明显高于假手术组(P<0.05);脑出血后第1天,抽吸治疗组NSE蛋白表达略高于脑出血组,但差异无统计学意义(P>0.05);脑出血后第3天、第5天抽吸治疗组与脑出血组比较,NSE蛋白表达明显降低(P<0.01)。结论Ⅳ型胶原酶诱导的大鼠脑出血模型是一种稳定、可靠的且能够实施立体定向血肿抽吸术的理想模型。大鼠脑出血后神经行为功能损伤,且血肿周围组织NSE蛋白表达明显增强。早期进行血肿抽吸术可以较好地清除脑内血肿,降低脑组织NSE蛋白的表达,减少脑出血后神经元的阳性细胞数及变性细胞数,从而保护神经组织,明显改善其神经行为功能。 Objective To investigate the changes of plasma neuron-specific enolase (NSE) in periosts after aspiration with stereotactic hematoma aspiration on the basis of type IV collagenase-induced experimental intracerebral hemorrhage. Methods Thirty-five male Wistar male rats were randomly divided into sham operation group, intracerebral hemorrhage group and suction treatment group. The rat model of intracerebral hemorrhage was established by injecting type Ⅳ collagenase into the rat caudate nucleus. After 10 h of intracerebral hemorrhage, 5 μl of a solution containing 3 000 U of urokinase was injected into the hematoma cavity to dissolve the blood clot. After 2 h, Stereotactic hematoma aspiration intervention treatment. The Bederson four-grade score was used to evaluate the neurological deficit in rats. The expression of NSE protein was detected by immunohistochemistry and Western blot respectively. Results After intracerebral hemorrhage in rats, NSE protein expression in cerebral hemorrhage group and aspiration group were significantly higher than those in sham operation group (P <0.05). On the first day after intracerebral hemorrhage, NSE protein (P> 0.05). Compared with cerebral hemorrhage group, the expression of NSE protein was significantly lower on the 3rd and 5th days after ICH (P <0.01) . Conclusion Type Ⅳ collagenase-induced intracerebral hemorrhage rat model is a stable and reliable model that can perform stereotactic hematoma aspiration. After intracerebral hemorrhage in rats, the neurobehavioral function was impaired and the expression of NSE protein in the perihematomal tissue was significantly increased. Early aspiration of hematoma can better clear the intracerebral hematoma, reduce the expression of NSE protein in brain tissue, reduce the number of neurons positive cells and degeneration after cerebral hemorrhage, so as to protect the nerve tissue and improve their neurobehavioral function.
