目的探讨基于计划-实施-检查-处理(plan-do-check-action,PDCA)循环管理的护理干预对头位异常产妇预后的影响。方法根据随机数字表法将2012年8月至2015年9月在解放军第309医院进行建档分娩的头位异常产妇124例分为观察组与对照组,每组各62例。对照组给予常规护理和特殊体位干预,观察组在对照组护理的基础上,对心理疏导、配合技巧和产程中呼吸方法指导、特殊体位干预进行PDCA循环管理,观察两组预后情况。结果观察组阴道分娩率高于对照组,产妇子痫、产后出血、胎盘早剥与新生儿窒息等不良预后情况明显少于对照组,观察组分娩健康知识掌握良好率和对护理的满意度评分明显高于对照组,差异均有统计学意义(P<0.05)。结论基于PDCA循环管理的护理干预有助于增加头位异常产妇对分娩期的知识掌握,提高阴道分娩率,改善母婴预后,使医患关系更加和谐。
Objective To explore the impact of nursing intervention based on plan-do-check-action (PDCA) cycle management on the prognosis of abnormal head maternal. Methods According to the random number table method, 124 cases of abnormal head maternal women who were filing and delivery in the 309th Hospital of PLA from August 2012 to September 2015 were divided into observation group and control group with 62 cases in each group. The control group was given regular nursing and special postural intervention. On the basis of the nursing care of the control group, the observation group conducted PDCA cycle management on psychological counseling, coordination skills, breathing method guidance and special postural intervention in the control group, and the prognosis of the two groups was observed. Results The vaginal delivery rate in the observation group was higher than that in the control group. Maternal eclampsia, postpartum hemorrhage, placental abruption and neonatal asphyxia had less adverse prognosis than those in the control group. The good rate of health knowledge and the satisfaction rating Which was significantly higher than that of the control group (P <0.05). Conclusions Nursing intervention based on PDCA cycle management can help to increase knowledge of maternal abnormal gestational hours during childbirth, improve vaginal delivery rate and improve maternal and infant prognosis, and make the doctor-patient relationship more harmonious.
