目的分析深圳市福田区疑似预防接种异常反应(AEFI)的发生特征。方法通过国家网络信息系统收集AEFI个案资料,采用描述性方法对相关指标进行分析。结果2009-2010年报告AEFI 394例,其中≤2岁儿童占65.74%,男女性别比为1.28∶1。接种后1 d内发生占79.44%。AFFI分类诊断中,一般反应占59.14%,异常反应占22.59%,偶合症占18.27%。临床诊断前3位为:发热、红肿、硬结,过敏性皮疹,无菌性脓肿,分别占57.11%、19.54%、5.08%。394例AEFI预后良好,未出现后遗症或死亡病例。发生AEFI前3位的疫苗为麻疹减毒活疫苗、全细胞百白破疫苗、甲型流感(裂解)疫苗,分别为69例(17.51%)、63例(15.99%)、57例(14.47%)。不同疫苗AEFI报告发生率在28.53/100万剂～1 495.00/100万剂。结论辖区AEFI监测敏感性和质量都较之前有明显的提高,各项监测指标均达到了国家方案要求,但AEFI报告发生率与WHO报道的预期发生率还有一定差距,说明监测工作仍有待提高。
Objective To analyze the occurrence characteristics of suspected immunization anaphylaxis (AEFI) in Futian District, Shenzhen. Methods AEFI case data were collected through the national network information system, and descriptive methods were used to analyze the related indicators. Results A total of 394 cases of AEFI were reported in 2009-2010, of which 65.74% were children ≤2 years old and the sex ratio was 1.28:1. Occurred within 1 d after inoculation accounted for 79.44%. AFFI classification diagnosis, the general reaction accounted for 59.14%, anomalous reactions accounted for 22.59%, coupling accounted for 18.27%. The first three cases of clinical diagnosis were fever, redness, induration, allergic rash and aseptic abscess, accounting for 57.11%, 19.54% and 5.08% respectively. 394 cases of AEFI good prognosis, no sequelae or deaths. The first 3 AEFI vaccines were live attenuated measles vaccine, whole-cell diphtheria-baptizee vaccine and influenza A (lytic) vaccine in 69 cases (17.51%), 63 cases (15.99%) and 57 cases (14.47% ). The incidence of AEFI in different vaccines ranged from 28.53 / 1 million doses to 1 495.00 / 1 million doses. Conclusion The sensitivity and quality of AEFI surveillance in the area are significantly higher than before. All monitoring indicators meet the requirements of the national program. However, there is still a gap between the reported incidence of AEFI and the expected incidence reported by the WHO, indicating that monitoring needs to be improved .
