目的 了解心脏自主神经功能与阵发性房性心动过速(PAT)发作和终止的关系。方法 对20例24hDCG捕捉到的PAT发作前与终止后的心脏自主神经功能状态,应用短时心率变异(HRV)时域、频域分析方法进行定量研究及非线性方法进行定性分析。结果 PAT发作前与终止后各项时域指标及频域指标中TP、VLF、LF、HF无显著性变化(P>0.05);PAT终止后IF_n和LF/HF比值均显著降低,HF_n显著升高(P<0.05)。PAT发作前与终止后RR间期散点图多呈彗星状,无明显变化。结论 PAT发作前与终止后存在着明显的自主神经功能变化,交感神经活性的增强与PAT发生明确有关,迷走神经张力的增高有利于PAT的自行终止。
Objective To investigate the relationship between cardiac autonomic nervous system function and the onset and termination of paroxysmal atrial tachycardia (PAT). Methods The autonomic nervous function of the heart was observed before and after the onset of paroxysmal PAT occlusion in 20 cases of 24h DCG. Quantitative study was conducted using time-domain and frequency-domain analysis of short-term heart rate variability (HRV) and the non-linear method was used for qualitative analysis. Results There was no significant change of TP, VLF, LF and HF in PAT before and after the onset of PAT (P> 0.05). After the termination of PAT, the ratios of IF_n and LF / HF were significantly decreased and HF_n significantly increased High (P <0.05). The scatter plots of RR interval before and after the onset of PAT were mostly comets, with no significant change. Conclusions There are obvious changes of autonomic nervous function before and after the onset of PAT. The enhancement of sympathetic nerve activity is clearly related to the occurrence of PAT. The increase of vagal nerve tension is helpful for the self termination of PAT.
