目的:对湖州市中心医院住院药房差错进行分析、干预,提高药品的调配质量。方法:对湖州市中心医院住院药房2014年的药房差错登记情况进行回顾性分析。结果:2014年上半年住院医嘱总计121500条,其中药房原因导致的差错2700件,占总发药医嘱的2.20%。2014年下半年住院医嘱总计117520条,其中药房原因的差错共900件,占总发药医嘱0.77%,同比差错率降低了1.43%。结论:通过对住院药房差错情况的分析与管理,有利于完善和建立药房多个环节的标准操作规程,提高药学人员的业务素质和责任心,防范药房的差错事件。
Objective: To analyze and intervene the errors of inpatient pharmacies in Huzhou Central Hospital to improve the blending quality of medicines. Methods: Retrospective analysis of pharmacy error registration in Huzhou Central Hospital in 2014 was conducted. Results: In the first half of 2014, a total of 121,500 inpatient orders were made, of which 2700 were due to pharmacy reasons, accounting for 2.20% of the total issued orders. In the second half of 2014, a total of 117,520 hospital doctors ’orders were made, of which 900 were causes of pharmacy errors, accounting for 0.77% of total dispensing doctors’ orders and a year-on-year decrease of 1.43%. Conclusion: Through the analysis and management of the errors in the inpatient pharmacies, it is beneficial to improve and set up the standard operating procedures in many aspects of the pharmacy to improve the professional quality and responsibility of pharmacy staff and prevent the error events in the pharmacy.
