探讨急性心肌梗塞早期血浆内皮素(ET)与心肌梗塞并发症、肌酸磷酸激酶(CPK)和左室射血分数的关系。应用放射免疫技术观察36例急性心肌梗塞患者(无并发症者22例为Ⅰ组,有并发症者14例为Ⅱ组)发病后72小时内血浆ET浓度变化,并选择22例健康志愿者作为对照。结果表明:Ⅰ组在发病后ET浓度升高很快,6小时内达峰值为52.08±4.34ng/L(P<0.05),但24小时内很快下降至正常水平;Ⅱ组浓度升高更快,6小时峰值为69.12±5.86ng/L(P<0.05),并持续到72小时仍维持较高水平;ET与CPK之间无明显相关性,且后者的峰值较晚;Ⅱ组ET与左室射血分数之间则呈负相关(P<0.05)。提示,急性心肌梗塞早期血浆ET素明显增高,对心肌血供及心脏功能有着很大的影响。
To investigate the relationship between early plasma endothelin (ET) and myocardial infarction complications, creatine phosphokinase (CPK) and left ventricular ejection fraction in acute myocardial infarction. Radioimmunoassay was used to observe the changes of plasma ET levels in 36 patients with acute myocardial infarction (22 patients without complications, group Ⅰ, 14 patients with complications) within 72 hours after onset, and 22 healthy volunteers Control. Tip, early acute myocardial infarction significantly increased serum ET, myocardial blood supply and cardiac function has a great impact.
