目的介绍一种肘关节外侧微小切口交叉克氏针体外持续加压内固定治疗儿童肱骨髁上骨折的方法和疗效。方法采用肘外侧切口对72例儿童肱骨髁上骨折外侧进行交叉内固定,并使针尾于体外张力相钩,形成外侧持续加压固定。结果骨折复位均良好,全部愈合,拔针时间3~4周,肘关节功能良好,3例发生轻度的内翻(<7°),但外观无影响,未作处理。结论肘关节外侧微小切口交叉克氏针体外持续加压固定治疗儿童肱骨髁上骨折具有创伤小、固定牢靠、功能恢复快、有利于骨折愈合,必要时外侧截骨,可有效地降低并发症的发生。
Objective To introduce a method and curative effect of supraclavicular supracondylar humerus fractures treated with continuous cross-over external fixation with Kirschner wire in the lateral elbow. Methods 72 cases of lateral supracondylar humerus fractures were treated by lateral incision of the elbow. The lateral pedicle of the supracondylar humerus was hooked to the outside of the body to form a continuous compression fixation. Results The fracture reduction was good and all of them were healed. The pulling time was 3 to 4 weeks. The elbow joint function was good and mild varus (<7 °) occurred in 3 cases. However, the appearance was not affected and was not treated. Conclusions Continuous supracondylar fractures of Kirschner wire with small incision outside the elbow can be used to treat supracondylar fractures of the humerus in children with less trauma, firm fixation and quick recovery of function, which is good for fracture healing and lateral osteotomy when necessary, which can effectively reduce the complications occur.
