目的探讨儿科重症感染合并多器官功能障碍综合征(MODS)患儿血清瘦素与细胞因子水平的联动效应及其意义。方法将2002-10—2004-03就诊于武汉市儿童医院和广州市妇婴医院的31例重症感染患儿与25名健康儿童分为研究组和对照组,其中所选的重症感染患儿均合并MODS。分别采用放射免疫法测定血清瘦素、酶联免疫吸附试验检测细胞因子白介素-6(IL-6)及肿瘤坏死因子-α(TNF-α)。结果研究组的血清瘦素水平及细胞因子IL-6及TNF-α水平较正常对照组明显升高,差异具有统计学意义;在研究组,血清瘦素与IL-6及TNF-α水平变化有统计学相关性。结论儿科重症感染合并MODS患者的血清瘦素与细胞因子水平明显升高,其联动效应是宿主抗感染的重要防御反应。监测血清瘦素与细胞因子水平的变化,对儿科重症感染的进程分析及预后判断具有重要临床意义。
Objective To investigate the synergistic effect of serum leptin and cytokines in pediatric severe infection complicated with multiple organ dysfunction syndrome (MODS) and its significance. Methods Totally 31 children with severe infectious diseases and 25 healthy children who were treated in Wuhan Children’s Hospital and Guangzhou Maternal and Child Hospital from October 2002 to April 2004 were divided into study group and control group. Among the selected children with severe infection Merge MODS. Serum leptin levels were determined by radioimmunoassay, and the levels of interleukin-6 (IL-6) and tumor necrosis factor-α (TNF-α) were detected by enzyme linked immunosorbent assay. Results The levels of serum leptin and the levels of IL-6 and TNF-α in the study group were significantly higher than those in the normal control group, and the difference was statistically significant. In the study group, the levels of serum leptin, IL-6 and TNF- There is a statistical correlation. Conclusions Serum levels of leptin and cytokines are significantly increased in patients with severe pediatric infection and MODS. The linkage effect is an important defensive response to host infection. Monitoring the changes of serum leptin and cytokines levels, pediatric severe infection process analysis and prognosis has important clinical significance.
