目的探讨骨髓象检验在急性白血病患者临床诊断中的应用情况。方法收集2013年—2016年治疗的120例急性白血病患者资料为观察组,均行骨髓象检验;另取120例同期健康体检者资料为对照组,比较两组检验结果和不同分型血象情况,计量资料组间比较采用检验,P<0.05为差异有统计意义。结果观察组白血病患者LIM抗癌结构蛋白1指标明显较对照组低,结缔组织生长因子、血管内皮生长因子-C、抗凋亡基因水平则较对照组高(χ~2=70.584、85.129、71.698、38.457,均P<0.05);M_1型血红蛋白水平较M_2、M_3型高(t=3.693、9.442,均P<0.05),且M_2型又较M_3型高(t=4.385,P<0.05);M_1、M_2型白细胞、血小板水平较M_3型高(t=3.251、2.830,均P<0.05),且M_1型血小板水平较M_2型低(t=6.301,P<0.05)。急性淋巴系白血病(acutelymphoblasticleukemia,ALL)血象分型中,L1型血红蛋白、白细胞水平较L2型高,血小板指标则较L2型低(χ~2=2.103、3.714、2.293,均P<0.05)。结论骨髓象检验能够明确诊断白血病患者骨髓细胞及分型血象具体情况,为患者治疗提供有效参考数据。
Objective To investigate the application of bone marrow imaging in clinical diagnosis of acute leukemia patients. Methods A total of 120 cases of acute leukemia treated in 2013-2016 were selected as the observation group. The bone marrow biopsy was performed. Another 120 healthy subjects were selected as the control group. The results of the two groups were compared with those of the different groups. Measurement data were compared between groups using the test, P <0.05 for the difference was statistically significant. Results The LIM anticancer protein 1 in observation group was significantly lower than that in control group. The levels of connective tissue growth factor, vascular endothelial growth factor-C and anti-apoptotic gene in observation group were higher than those in control group (χ ~ 2 = 70.584, 85.129, 71.698 , 38.457, all P <0.05). The M_1 hemoglobin level was higher than that of M_2 and M_3 (t = 3.693,9.442, P <0.05), and M_2 was higher than M_3 (t = 4.385, P <0.05). M_1, M_2 leucocytes and platelets were higher than M_3 (t = 3.251,2.830, all P <0.05), and M_1 platelet was lower than M_2 (t = 6.301, P <0.05). In the acute lymphoblastic leukemia (ALL) genotypes, L1 hemoglobin and leukocyte levels were higher than L2 and platelet indexes were lower than L2 (χ ~ 2 = 2.103, 3.714, 2.293, all P <0.05). Conclusion Bone marrow imaging can clearly diagnose the bone marrow cells in patients with leukemia and the specific situation of blood typing, providing effective reference data for the treatment of patients.
