目的:对比肺癌粟粒样脑转移瘤的T1WI/FFE增强序列与T1WI/SE增强序列的诊断效率及图像质量,探讨T1WI/FFE增强序列在脑多发粟粒样转移瘤中的价值。方法:应用3.0 T MR机对本院2011年7月至2012年6月期间住院的20例病理确诊肺癌临床资料证实为多发脑转移瘤的在MRI平扫时无明显异常信号的患者,进行T1WI/SE增强及T1WI/FFE增强扫描,观察两序列对微小病灶(<3 mm)的显示能力;同时测量、比较病灶的信噪比(SNR),对比度噪声比(CNR),脑灰白质的对比度噪声(CNR)。结果:20例粟粒样脑转移患者中,所有病例均显示为结节状、粟粒状及小环形强化。全部病灶均能被T1WI/FFE增强序列及T1WI/SE增强序列显示,对微小病灶的显示能力T1WI/FFE序列的主观评分为3.6±0.4,T1WI/SE序列的主观评分为2.7±0.4。T1WI/FFE增强序列脑灰白质对比度噪声高于T1WI/SE序列(CNR:30±5.22 vs 20±4.28),两者之间有统计学意义(P<0.05)。T1WI/FFE序列的扫描时间为1 min36s,T1WI/SE序列的扫描时间为3 min 20 s(P<0.01)。T1WI/FFE序列SNR与T1WI/SE序列SNR,两者之间无统计学差异(P>0.05)。T1WI/FFE序列CNR与T1WI/SE序列CNR,两者之间无统计学意义(P>0.05)。结论:T1WI/FFE序列与T1WI/SE序列相比,更能够清晰显示微小病灶及脑灰白质对比,且扫描时间明显缩短。
OBJECTIVE: To compare the diagnostic efficiency and image quality of T1WI / FFE enhancement sequence and T1WI / SE enhancement sequence of lung miliary brain metastases and to investigate the value of T1WI / FFE enhancement sequence in multiple brain metastases. Methods: The clinical data of 20 patients admitted to our hospital from July 2011 to June 2012 admitted to our hospital from July 2011 to June 2012 were retrospectively analyzed. The subjective score of T1WI / FFE sequence for subtle lesions was 3.6 ± 0.4, and the subjective score of T1WI / SE sequence was 2.7 ± 0.4. Compared with the T1WI / SE sequence (CNR: 30 ± 5.22 vs 20 ± 4.28), the T1WI / FFE enhancement sequence had a higher contrast of brain gray matter than that of the T1WI / SE sequence (P <0.05).
