目的:了解牙周炎患者高血压前期的发病情况,分析牙周炎的严重程度与高血压前期的关系,探讨炎性细胞因子在牙周炎伴高血压前期中的作用。方法:对310例牙周炎患者进行血压测量和牙周检查,调查牙周炎患者高血压前期的发病情况,分析轻、中、重度牙周炎与高血压前期的关系;随机选择单纯牙周炎43例,牙周炎伴高血压前期40例,伴高血压38例,测定受试者血清中的超敏C反应蛋白(hs-CRP)、白细胞介素-6(IL-6)和肿瘤坏死因子(TNF-α)水平,分析上述指标之间的差异。结果:牙周炎患者中高血压前期的发病率为42.26%,男性高于女性(P<0.01);中、重度牙周炎伴高血压前期的发病率分别为36.64%和42.75%,显著高于轻度牙周炎伴高血压前期的发病率(P<0.05或<0.01);牙周炎伴高血压前期组的血清hs-CRP、IL-6、TNF-α水平显著高于单纯牙周炎组(P<0.05或P<0.01);多元逐步回归分析显示,中、重度牙周炎与高血压前期呈独立相关(P<0.05),血清hs-CRP、IL-6、TNF-α水平与牙周炎伴高血压前期呈独立相关(P<0.05)。结论:牙周炎患者高血压前期的发病率较高;炎性细胞因子与牙周炎伴高血压前期密切相关。
Objective: To understand the incidence of prehypertension in patients with periodontitis, analyze the relationship between the severity of periodontitis and prehypertension, and explore the role of inflammatory cytokines in periodontitis with prehypertension. Methods: 310 patients with periodontitis were measured blood pressure and periodontal examination to investigate the incidence of prehypertension in patients with periodontitis, the relationship between light, moderate and severe periodontitis and prehypertension; random selection of simple periodontal 43 cases of inflammation, 40 cases of periodontitis with prehypertension and 38 cases of hypertension. The serum levels of hs-CRP, IL-6 and tumor Necrosis factor (TNF-α) levels, analysis of the differences between the above indicators. Results: The incidence of prehypertension in patients with periodontitis was 42.26%, which was higher in males than in females (P <0.01). The incidence rates of moderate and severe periodontitis with prehypertension were 36.64% and 42.75% respectively, which were significantly higher than The incidence of mild periodontitis with prehypertension (P <0.05 or <0.01); the levels of hs-CRP, IL-6 and TNF-α in periodontitis with prehypertension group were significantly higher than those in simple periodontitis (P <0.05 or P <0.01). Multivariate stepwise regression analysis showed that moderate and severe periodontitis was independently associated with prehypertension (P <0.05), serum hs-CRP, IL-6 and TNF- Periodontitis was independently associated with prehypertension (P <0.05). Conclusion: The incidence of prehypertension in patients with periodontitis is high. Inflammatory cytokines are closely related to periodontitis with prehypertension.
