国外自1906年 Mackenzie 氏首先报告1例严重心力衰竭的病人在吞咽时发生房室传导阻滞后,关于吞咽时发生心律失常和晕厥的个案报告,至1980年计有14例。国内张克智等共计报道7例。我们以中西医结合诊治1例,现报告如下:王××,男,37岁,电工,上虞县人。80年2月有心窝部阵发性剧痛伴呕吐蛔虫史。80年4月22日晨,于叠被时突然心悸、胸闷、晕倒,经厂卫生科心电图检查,诊为房颤。5月2日起,自觉从早到晚每于吞咽时即心悸,以早餐为甚,常可使心悸持续1～4小时。浙医大附属一院心电图检查为:窦性心律不齐、频发房早呈短串房性心动过速伴文氏型房室传导,随吞咽动作而发生(见图)。8月26日至9月26日住某医院治疗。入院后 Holter 动态心电图观察23小时48分钟,心律情况为:连续3次以上房早形 Abroad Since 1906 Mackenzie’s first report of a case of severe heart failure in patients with atrioventricular block occurred after swallowing, arrhythmia and syncope occurred when swallowing case reports, to the count in 14 cases in 1980.
