目的 本文对 7例骨原发性恶性淋巴瘤 (PrimaryLymphomaofBone ,PLB)进行免疫组化及病理形态定量测定研究。方法 对瘤组织采用常规石蜡切片、HE染色及ABC法免疫组化染色 ,光镜观察。应用武汉同济医科大学千屏影像公司HPIAS - 10 0 0彩色图像分析系统对肿瘤细胞核的形态进行定量测定。结果 7例中B小淋巴细胞型 2例 ,小裂核型 2例 ,大无裂型 1例 ,多形T型 1例 ,组织细胞型 1例。形态定量测定结果表明 5种类型肿瘤细胞核的 15项形态学参数与对照组之间具有高度显著性 (P <0 .0 0 1)或显著性差异 (P <0 .0 5)。各型淋巴瘤细胞核的 15项形态学参数之间亦大多数具有高度显著性 (P <0 .0 0 1)或显著性差异 (P <0 .0 5)。结论 形态定量测定的方法对于PLB的正确诊断具有重要的参考价值 ,并对PLB的组织学分型能够提供客观的、准确的定量依据 ,因而具有十分重要的意义。其对于恶性淋巴瘤患者预后的判断亦可能具有重要意义。
Objective To investigate the immunohistochemical and pathological morphological features of 7 cases of primary lymphoma of primary bone (PLB). Methods The paraffin sections, HE staining and ABC immunohistochemical staining were performed on the tumor tissues and observed under light microscope. The quantitative analysis of tumor cell nucleus was performed using the HPIAS-1000 color image analysis system of Qianjin Imaging Company, Wuhan Tongji Medical University. Results Among the 7 cases, there were 2 cases of B small lymphocyte type, 2 cases of small split karyotype, 1 case of large non-split type, 1 case of polymorphic T type, and 1 case of histiocytic type. Quantitative morphometric analysis showed that the 15 morphological parameters of the five types of tumor nucleus were highly significant (P < 0.01) or significant differences (P <0.05) from the control group. Most of the 15 morphological parameters of all types of lymphoma cells were highly significant (P < 0.01) or significant differences (P <0.05). Conclusion Quantitative quantitative method has important reference value for the correct diagnosis of PLB, and it can provide objective and accurate quantitative basis for the histological classification of PLB, so it has very important significance. Its assessment of the prognosis of patients with malignant lymphoma may also be of significance.
