目的观察一半汤对IgA肾病(IgAN)大鼠蛋白尿及Nephrin、CD2AP表达的影响。方法将56只SPF级Wistar雄性大鼠随机抽取10只作空白组,其余46只造模。采用牛血清白蛋白(BSA)+四氯化碳(CCl4)+脂多糖(LPS)的方法建立IgAN大鼠模型,持续12周,将造模完成后所剩的40只大鼠随机分为模型组、肾炎康复片组、二半汤组、一半汤组,每组各10只。肾炎康复片组、二半汤组、一半汤组分别给予肾炎康复片、二半汤、一半汤灌胃,持续8周。分别于造模第12周,治疗第4,6,8周末测24h尿蛋白定量(24h-UPr),尿常规。治疗第8周末处死大鼠心脏取血,检测血肌酐(SCr)、尿素氮(BUN),取大鼠肾皮质检测Nephrin、CD2APmRNA及相关蛋白的表达。结果造模完成后,模型组24h-UPr和尿红细胞计数较空白组显著增多(P<0.05);治疗完成后,肾炎康复片组、二半汤组、一半汤组24h-Upr较模型组显著降低(P<0.05),一半汤组较肾炎康复片组显著降低(P<0.05);肾炎康复片组、二半汤组、一半汤组之间红细胞计数无统计学差异(P>0.05);肾炎康复片组、二半汤组、一半汤组SCr,BUN较模型组显著降低(P<0.05),一半汤组较肾炎康复片组显著降低(P<0.05)。肾炎康复片组、二半汤组、一半汤组Nephrin、CD2APmRNA及相关蛋白的表达较模型组显著降低(P<0.05),一半汤组较肾炎康复片组显著降低(P<0.05)。结论一半汤能降低尿蛋白,保护肾功能,其作用可能与调节Nephrin、CD2AP的表达有关,且疗效较二半汤进一步提高。
Objective To observe the effect of half soup on proteinuria and Nephrin and CD2AP expression in IgA nephropathy rats. Methods Sixty-six male Sprague-Dawley (Wistar) male rats were randomly divided into blank group (n = 10) and the remaining 46 models. IgAN rat model was established by BSA + CCl4 + LPS for 12 weeks, and the remaining 40 rats were randomly divided into model Group, nephritis rehabilitation tablets group, two half soup group, half soup group, each group of 10. Nephritis rehabilitation tablets group, two half soup group, half soup group were given nephritis rehabilitation tablets, two half-soup, half soup gavage for 8 weeks. Respectively in the modeling of the first 12 weeks, the treatment of 4,6,8 weekend measured 24h urine protein (24h-UPr), urine routine. At the end of the 8th week, rats were sacrificed and their hearts were sacrificed. Blood samples were collected for determination of serum creatinine (SCr), blood urea nitrogen (BUN), and renal cortices. Nephrin, CD2AP mRNA and related proteins were detected. Results After the model was established, the 24h-UPr and urinary erythrocyte count in the model group were significantly higher than those in the blank group (P <0.05). After the treatment, the 24h-Upr of the Shenyankang tablet group, the two half-soup group and the half- (P <0.05). The serum level of Erythrocyte in Shiyan decoction group was significantly lower than that of Shenyinkang decoction group (P <0.05). There was no significant difference in the count of erythrocytes between Shenyankang tablet group, Erban decoction group and Baotang decoction group (P> 0.05) The levels of SCr and BUN in Shenyankang Tablet, Erhuan Decoction group and half-soup group were significantly lower than those in model group (P <0.05), while those in Shenhuang Tablet group were significantly lower than those in Shenyankang Tablet group (P <0.05). The expression of Nephrin, CD2APmRNA and related proteins in Shenyanyang Tablet group, Erhuan Tang group and Banxuexutang group were significantly lower than those in model group (P <0.05), while those in Shenhuang Tablet group were significantly lower than those in Shenyinkang Tablet group (P <0.05). Conclusion Decoction can reduce urinary protein and protect renal function, which may be related to the regulation of Nephrin and CD2AP expression, and the curative effect is further improved than that of the second half soup.
