背景:偏瘫患者日常生活能力的恢复一定程度上涉及到运动能力的恢复情况,但两者是不是有高度的相关性,既往的研究没有定论。目的:探讨脑卒中偏瘫患者上、下肢运动能力与日常生活能力的相关性。设计:病例分析。单位:南京医科大学第一附属医院康复科。对象:选择2002-08/2003-06在江苏省人民医院康复科住院的脑卒中偏瘫患者30例。男20例,女10例;年龄39～76岁。方法:采用简易Fugl-Meyer运动功能评分方法评定患者运动能力,改良Barthel指数评定患者的日常生活能力。主要观察指标:①脑卒中偏瘫患者运动功能评分和改良Barthel指数评分。②脑卒中偏瘫患者运动能力与日常生活能力的相关性。结果:30例患者进入结果分析。①全身Fugl-Meyer运动功能评分平均为40.96±32.04,上肢运动功能评分平均为21.78±22.87,下肢运动功能评分平均为18.43±10.29。改良Barthel指数评分平均为39.46±25.94。②全身总Fugl-Meyer运动功能评分与改良Barthel指数评分的相关系数为0.65,其中上肢运动功能评分与改良Barthel指数评分的相关系数为0.62,下肢运动功能评分与改良Barthel指数评分的相关系数为0.61。结论:偏瘫患者的运动能力与日常生活能力有一定的相关性,且上、下肢运动能力对日常生活能力的影响几乎相等。
Background: The recovery of daily living ability of patients with hemiplegia relates to the recovery of motor ability to a certain extent. However, the correlation between the two is not high, and the previous studies are inconclusive. Objective: To investigate the correlation between upper and lower extremity motor ability and daily living ability in patients with stroke and hemiplegia. Design: Case Analysis. Unit: First Affiliated Hospital of Nanjing Medical University rehabilitation department. PARTICIPANTS: Thirty patients with stroke and hemiplegia who were hospitalized in the rehabilitation department of Jiangsu Provincial People ’s Hospital from August 2002 to June 2003 were selected. 20 males and 10 females; aged 39 to 76 years old. Methods: The simple Fugl-Meyer motor function score method was used to assess the patient’s motor ability. The modified Barthel index was used to assess the patients’ daily living ability. MAIN OUTCOME MEASURES: ① Motor function score and modified Barthel index score in patients with stroke hemiplegia. ② stroke hemiplegia patients with motor ability and daily living ability. Results: Thirty patients entered the result analysis. ① The average body function score of Fugl-Meyer was 40.96 ± 32.04, the average score of upper limb motor function was 21.78 ± 22.87, and the average score of lower limb motor function was 18.43 ± 10.29. The modified Barthel index averaged 39.46 ± 25.94. (2) The correlation coefficient between general motor score of Fugl-Meyer motor function and modified Barthel index was 0.65. The correlation coefficient between upper motor function score and modified Barthel index was 0.62. The correlation coefficient between lower motor function score and modified Barthel index was 0.61 . Conclusion: The hemiplegia patients’ exercise ability has some correlation with the daily living ability, and the motor ability of upper and lower limbs has almost equal impact on their daily living ability.
