目的:观察IGF-II、IGF-IR在结肠腺瘤中的表达,探讨其生物学意义。方法:免疫组化方法分别检测IGF-II、IGF-IR、PCNA在正常结肠粘膜组(A组,n=20)、低级别上皮内瘤变组(B组,n=50)、高级别上皮内瘤变组(C组,n=50)和结肠癌组(D组,n=20)中的表达,TUNEL法检测细胞凋亡。结果:IGF-II、IGF-IR、PCNA在结肠癌组和高级别上皮内瘤变组中呈高表达,两者之间无显著差异,分别与低级别上皮内瘤变组、正常大肠粘膜组相比表达有显著差异;低级别上皮内瘤变组、高级别上皮内瘤变组、结肠癌组的凋亡指数(AD)逐渐降低,且两两比较有显著差异。IGF-II、IGF-IR的表达与增殖指数(PI)成正相关、与AI成负相关;PI、AI在低级别上皮内瘤变组IGF-II、IGF-IR阴性和阳性中无显著性差异,在高级别上皮内瘤变组IGF-II、IGF-IR阴性和阳性中有显著差异。结论:IGF-II、IGF-IR的异常表达与结肠腺瘤癌变密切相关;IGF-II、IGF-IR促进细胞片常增殖,抑制肿瘤细胞凋亡,在结肠腺瘤痛变过程中起了重要的作用;IGF-II、IGF-IR可作为结肠癌的预测指标运用于临床。
OBJECTIVE: To observe the expression of IGF-II and IGF-IR in colorectal adenoma and to explore its biological significance. Methods: Immunohistochemistry was used to detect the expression of IGF-II, IGF-IR and PCNA in normal colonic mucosa (group A, n = 20), low grade intraepithelial neoplasia (group B, n = 50) (C group, n = 50) and colon cancer group (D group, n = 20). The apoptosis was detected by TUNEL method. Results: The expressions of IGF-II, IGF-IR and PCNA in colon cancer and high-grade intraepithelial neoplasia were significantly higher than those in low-grade epithelial neoplasia, normal colorectal mucosa (P <0.05). The apoptotic index (AD) of low-grade intraepithelial neoplasia group, high-grade intraepithelial neoplasia group and colon cancer group decreased gradually, and there was a significant difference between every two groups. The expression of IGF-II and IGF-IR was positively correlated with proliferation index (PI) and negatively correlated with AI. There was no significant difference between PI and AI in IGF-II, IGF-IR negative and positive in low-grade intraepithelial neoplasia , There was a significant difference in IGF-II, IGF-IR negative and positive in high-grade intraepithelial neoplasia group. Conclusions: The abnormal expression of IGF-II and IGF-IR is closely related to the carcinogenesis of colonic adenomas. IGF-II and IGF-IR can promote the proliferation of cells and inhibit the apoptosis of colorectal adenoma, which play an important role in the process of colorectal adenoma The role of IGF-II, IGF-IR as a predictor of colon cancer clinical application.
