[目的]探讨垂体腺瘤患者的中心视野的改变特点,为早期诊断、及时治疗提供依据。[方法]应用OCTOPUS101自动视野仪对122例垂体腺瘤患者进行中心视野检查的视野进行回顾性分析。[结果]122例垂体腺瘤患者中,男女性发病比例为1.0︰0.7;发病年龄8～81岁,平均年龄44.5岁。122例垂体腺瘤中,视野改变者96例(78.69%)。双眼视野表现为双眼颞侧偏盲28例(22.95%);双眼颞上象限陷入11例(9.02%);1眼颞侧偏盲,另眼全视野缺损9例(7.34%);1眼视野正常,另眼颞上象限偏盲,双眼管状视野各5例(4.10%);双眼颞上象限性偏盲,1眼颞上象限性偏盲、另眼视野正常各4例(3.28%);双眼同侧偏盲3例(2.45%);少见的视野表现如鼻侧视野偏盲,中心、旁中心暗点、弓形暗点、水平偏盲及生理盲点扩大等表现合计27例(22.13%)。[结论]垂体腺瘤多数视野改变可表现为双颞侧偏盲(双颞侧偏盲或双颞侧上象限偏盲),不常见的为单眼颞侧偏盲、双眼管状视野、同侧偏盲、中心暗点或旁中心暗点、弓形暗点等。
[Objective] To investigate the characteristics of central visual field changes in patients with pituitary adenoma and provide basis for early diagnosis and timely treatment. [Methods] The visual fields of 122 patients with pituitary adenoma who underwent central visual field examination were retrospectively analyzed by OCTOPUS101 automatic visual field instrument. [Results] Among the 122 pituitary adenoma patients, the incidence of male and female was 1.0: 0.7. The age of onset was from 8 to 81 years with a mean age of 44.5 years. Of the 122 pituitary adenomas, 96 (78.69%) had visual field changes. The binocular visual field showed binocular temporal hemianopsia in 28 cases (22.95%); the binocular temporal superior quadrant was found in 11 cases (9.02%); the temporal hemianopsia in one eye and the other 9 cases (7.34% (5 eyes) (4.10%) in the other upper quadrant of the superior temporal quadrant, and 5 eyes (4.10%) in each of the binocular and tubular eyes. The bilateral upper quadrant quadrant hemianopia, Cases (2.45%); rare visual field performance such as nasal visual field partial hemianopia, center, next to the center of dark spots, arched dark spots, horizontal hemianopia and physiological blind spots to enlarge the performance of a total of 27 cases (22.13%). [Conclusion] The majority of visual field changes of pituitary adenomas can be manifested as bilateral temporal hemianopia (bilateral temporal hemianopia or bilateral temporal quadrant hemianopia), uncommon for unilateral temporal hemianopsia, binocular tubular vision, ipsilateral hemianopia, central dark spots Or next to the center dark spots, bow-shaped dark spots and so on.
