不同体位对咽鼓管功能影响的研究不多。文献中谓当头向前屈或平卧位时,鼻咽部需要有较高的压力才能使咽鼓管开放,是因在此体位时,围绕管口粘膜的静脉流体压力增高,淋巴郁积,使咽鼓管的通畅度减低。作者用声管测量法,测试正常人及感冒患者体位改变与咽鼓管功能的关系,并观察体位改变到什么程度才能影响它的功能。测试对象包括34名健康人,男性25,女性9,平均年龄35岁(21~56岁),2名咽鼓管功能正常的感冒患者。声管测量依次用6、7和8kHz音,被测试者取头直位,先饮水一次,然后吞咽唾液,每个测试音反复作同样动作三次,以测定咽鼓管的通畅性。34例中选择61耳咽鼓管通畅者作为观察对象。每个被测试者取4个体位:(1)坐式头直位;(2)仰卧头抬高20°;(3)仰卧头平位;(4)俯卧头平位。每个被测试者在4种不同的体位上,反复测试3次,为了避免观察的相互依赖性,34例测试对象都随机抽查一耳,将测量值作统计学处理(秩积检验法)未发现体位对咽鼓管功能有影响。但存在个体差异性,如有些人在卧位时反较坐位更易使咽鼓管开放,只有2例在俯卧位时不能使咽鼓管张开,但在其他体位均能使管张开;2例鼻咽炎患者,各种体位均能使咽鼓管开放。声管测量法是一种生理性试验,当吞咽使咽鼓管张开时,它能绘出一个可以信赖的图形,而且当鼻咽炎在发生或消退时,能灵敏地测出其改变。本研究结果说明,体位对咽鼓管功能的影响不像以往想像的那样重要,也不支持从前的那种看法,认为卧位时应将头抬高20°,可以防止发生中耳炎或促进中耳炎症的吸收。
There are not many studies on the eustachian tube function in different positions.
