男,24岁。左颌下囊性肿物五年,始枣大,不痛,消炎无效入院,住院号240182。查左颌下鸡蛋大肿物,界清,与皮肤无粘连,颌下腺导管开口正常。术中见肿物位于颌下腺内,界清,颌下腺受压仅余3.5×3×2cm狭条,诊为混合瘤全部切除送检。病理检查:45克园形肿物5.5×4×3.5cm,包膜完整,中等硬,切面见大小不等内含淡黄皮脂样物的薄壁囊腔。光镜:纤维包膜内由位于致密结缔组织内的囊肿群、脂肪组织及慢性萎缩性炎的颔下腺组成。囊壁外为纤维组织,内衬单、复层鳞状上皮、皮脂腺或多核巨细胞,后者有丰富紫红包含空泡的胞浆,核数个或数十个,中或偏位,不整或菊形团样排列,囊腔空
