目的:对门诊患者进行跌倒风险评估,按评估结果进行分级干预及综合预防,减少跌倒发生,保证门诊患者安全。方法:对重点评估人群使用昆士兰大学跌倒危险因素评估表,进行跌倒风险筛查,量化跌倒风险评估系统,根据评估结果提供安全门诊环境,分级干预,加强健康宣教,制定应急预案,全员参与等针对性管理措施,预防跌倒发生。结果:实施门诊患者跌倒风险筛查及综合抗跌倒护理对策后,门诊患者跌倒例数由2015年1~6月的6例次,降到2015年7~12月的0例次,护士考核合格率100%,患者防跌倒认知水平及防跌倒护理满意度明显提高,与实施前比较差异有统计学意义。结论:门诊患者跌倒重在预防,通过跌倒风险筛查及护理对策,可有效预防和减少跌倒不良事件的发生。
OBJECTIVE: To assess the risk of falls in outpatients, and to make grading intervention and comprehensive prevention according to the assessment results to reduce the incidence of falls and ensure the safety of outpatients. Methods: To use the University of Queensland fall risk factor assessment form to evaluate the fall risk and quantify the fall risk assessment system for the key assessment population, to provide a safe outpatient environment, grading intervention, strengthen health education, develop contingency plans, full participation and so on according to the assessment results Targeted management measures to prevent falls. Results: After outpatient risk screening and comprehensive anti-fall care strategies were implemented, the number of outpatients falling from 6 cases in January-June 2015 to 0 cases in July-December 2015, the nurses passed the examination The rate of cognition of anti-fall patients and the satisfaction degree of fall prevention nursing care were significantly increased, with significant difference compared with before implementation. Conclusion: Outpatient falls mainly in the prevention of falls. By means of risk screening of falls and nursing countermeasures, it can effectively prevent and reduce the incidence of falls adverse events.
