患者男性,72岁。因右足底硬结约五年,行走、负重时疼痛月余就诊。检查:右足底有一角化层硬结约3.5×2 cm,局部未见有色素沉着,按压疼痛,表面无糜烂及渗出。在门诊局麻下行常规鸡眼切除术。术后十天折线,创口全部裂开,十天来好转而收住外科治疗。发现创面有黑色渗出液。将局部组织送检。病理报告为恶性黑色素瘤。进一步体检时发现右侧腹股沟有5个约栗子～花生仁大小的淋巴结,活动尚好。行右侧腹股沟淋巴结切除术,术中见分离的淋巴结呈青紫
The patient is male, 72 years old. Due to the induration of the right plantar foot for about five years, more than a month’s pain occurred during walking and weighting. Examination: The right foot had an keratinized layer with an induration of about 3.5 × 2 cm. There was no local hyperpigmentation, no pain, and no erosion or exudation on the surface. Black exudate was found on the wound. The local organization will be sent for inspection. The pathology report was malignant melanoma. On further physical examination, there were 5 lymph nodes of approximately chestnut to peanut size in the right groin, and the activity was still good. The right inguinal lymph node resection was performed. During the operation, the isolated lymph nodes showed cyanosis.
