[Methods] The MSM population was recruited and self-signed anonymously by respondent-driven recruitment (RDS) method. Blood samples were collected for biological testing. [Results] A total of 201 people were surveyed, with homosexuality accounting for 75.6% and bisexual (BI) accounting for 13.4%. Small groups and internet activities were the mainstay. The total awareness rate of basic knowledge of STD / AIDS was 68.6%. There were significant differences among different cultures, occupations, ages, matrimonies, incomes and sexual orientation. The upward trend was significant with the increase of culture, age and income. In the last 6 months: 16.4% for male commercial partners (MB), 39.3% for each condom use, 68.2% for fixed male partners (BF) and 24.8% for each condom use, and Male occasional sexual partners (419) accounted for 37.8% of the sex, 30.3% per use of condoms, condom use rate increased with the STD / AIDS prevention and control knowledge awareness is on the rise. Biological monitoring of 81 people, RPR positive rate of 8.6%, TPPA positive rate of 14.8%, found no HIV (HIV), hepatitis C infection. [Conclusion] The MSM in Mianyang City have complicated sexual life, low awareness rate of STD / AIDS prevention and control, unsafe sex, high syphilis infection rate and potential threat to AIDS epidemic. Current favorable opportunities should be seized to develop interventions that are tailored to the characteristics of the local MSM population and to promote safety behaviors to reduce the risk of AIDS infection.
