目的 对抽样人群脑血管血液动力学指标 (CVHI)检测和分析 ,研究中国正常人CVHI的参考值范围。方法 在进行中风预防干预前瞻性研究的同时 ,从东北、华北、西南、西北、华东、华南等六大行政区 ,2 6个城市筛选了无脑卒中病史和危险因素暴露史的正常对照者 1 4 4 80名 (3~ 92岁 ) ,进行CVHI指标检测。将检测结果按照年龄、性别、地区和左右侧检测部位分组 ,用正态分布法计算单侧 95 %参考值范围。结果 1 0对检测指标的 95 %参考值范围的分布特点是 :颈总动脉血流速度的最大值 (Vmax)、最小值 (Vmin)、平均值 (Vmean)的下限随着年龄的增长呈下降趋势 ,而脑血管外周阻力 (RV)、特性阻抗 (Zcv)、脉搏波波速 (WV)、动态阻力 (DR)、临界压 (CP)等指标的上限则随着年龄的增长呈上升趋势 ,血流量 (Qmean)和压力差 (DP)无明显的年龄升降趋势 ;男性的Qmean、Vmax的下限和RV、DR、CP、DP的上限高于女性 ,而WV和Zcv的上限低于女性 ;左侧的Qmean、Vmean、Vmax、Vmin上限和WV、Zcv、CP的下限高于右侧 ,而RV、DR和DP的上限低于右侧 ;不同地区间各指标的分布差异无显著性。结论 不同年龄、性别和左右检测部位CVHI检测指标的单侧 95 %参考值范围有明显差异 ,参考值范围应分组计算。
Objective To detect and analyze the cerebrovascular hemodynamic parameters (CVHI) in sampled population and to study the reference range of CVHI in Chinese normal subjects. Methods In the prospective study of prevention of stroke, we also screened out the control subjects with no history of stroke and exposure to risk factors from 26 cities in Northeast China, North China, Southwest China, Northwest China, East China and South China 14 4 80 (3 to 92 years old), CVHI index test. The test results were grouped according to age, gender, region and the left and right side of the test site, using the normal distribution method to calculate 95% of the unilateral reference range. Results The distribution of 95% of the reference value of the detection indexes was: the maximum (Vmax) and minimum (Vmin) of the common carotid artery blood flow velocity and the lower limit of the mean (Vmean) decreased with the increase of the age While the upper limit of cerebral vascular resistance (RV), characteristic impedance (Zcv), pulse wave velocity (WV), dynamic resistance (DR) and critical pressure (CP) increased with the increase of age Qmean and DP had no obvious age-increasing trend; the lower limit of Qmean and Vmax and the upper limit of RV, DR, CP and DP were higher in men than in women, while the upper limit of WV and Zcv was lower than that in women; The upper limit of Qmean, Vmean, Vmax, Vmin and WV, Zcv, CP lower than the right, and the upper limit of RV, DR and DP lower than the right; the distribution of each index between different regions was not significant. Conclusion The 95% unilateral 95% reference range of the CVHI test scores of different age, gender and left and right test sites have significant differences, and the reference range should be calculated in groups.
