目的观察良性前列腺增生(BPH)患者行手术治疗可能存在的代谢因素。方法应收集2013年11月~2015年7月,在新疆医科大学第一附属医院住院明确诊断为BPH的患者575例,分为手术组(508例)及非手术组(67例)。对每位入选者进行问卷调查,包括年龄、民族、BPH年限、吸烟饮酒情况、I-PSS评分、QOL评分,以及是否有高血压病史、糖尿病病史、高脂血症,并测量身高、体重、腰围、血压情况,抽血化验血脂、血糖水平,行B超检查得出前列腺体积。应用单因素相关分析和多因素非条件Logistic回归分析方法对有关因素进行分析,了解影响BPH患者治疗方式的可能危险因素。结果单因素分析结果显示:腰围、HDL、前列腺体积、I-PSS评分、QOL评分、高血压病、代谢综合征与前列腺增生行手术治疗存在相关性(P<0.05),经多因素非条件Logistic回归分析结果表明:前列腺体积大、高血压病、I-PSS评分高、患代谢综合征可增加BPH患者的手术几率(OR值分别为2.140、2.371、3.822、3.891,95%CI分别为0.999~1.022、1.215~4.627,1.019~1.169,0.982~6.152,均为P<0.05)。结论腰围、HDL与BPH患者行手术治疗呈正相关;前列腺体积大、高血压病、I-PSS评分高、患代谢综合征的BPH患者更可能采取手术方式治疗BPH。
Objective To observe the possible metabolic factors in the surgical treatment of benign prostatic hyperplasia (BPH). Methods From November 2013 to July 2015, 575 patients diagnosed as BPH in the First Affiliated Hospital of Xinjiang Medical University should be collected, divided into operation group (508 cases) and non-operation group (67 cases). A questionnaire survey was conducted on each participant, including age, ethnicity, BPH duration, smoking and drinking status, I-PSS score, QOL score, history of hypertension, history of diabetes, hyperlipidemia and height, weight, Waist circumference, blood pressure, blood test blood lipids, blood glucose levels, line B ultrasound examination of the prostate volume. Univariate correlation analysis and multivariate non-conditional logistic regression analysis were used to analyze the relevant factors to understand the possible risk factors influencing the treatment of BPH patients. Results The results of univariate analysis showed that waist circumference, HDL, volume of prostate, I-PSS score, QOL score, hypertension and metabolic syndrome were associated with surgical treatment of benign prostatic hyperplasia (P <0.05) Regression analysis showed that: Prostate volume, hypertension, high I-PSS score, patients with metabolic syndrome can increase the risk of surgical BPH patients (OR = 2.140,2.371,3.822,3.891,95% CI 0.999 ~ 1.022, 1.215 ~ 4.627, 1.019 ~ 1.169, 0.982 ~ 6.152, all P <0.05). Conclusion The waist circumference and HDL are positively correlated with the surgical treatment of BPH patients. BPH patients with large prostate volume, hypertension, high I-PSS score and metabolic syndrome are more likely to be surgically treated with BPH.
