目的探讨几种内分泌疾病低钾血症的发生率与诊断、治疗措施。方法对58例内分泌疾病引起的低钾血症进行回顾性分析。结果59例中15例由原发性醛固酮增多症引起,平均年龄41.0岁,低血钾发生率为100%;15例由柯兴氏综合征引起,平均年龄36.8岁,低血钾发生率为47%;4例由嗜铬细胞瘤引起,平均年龄42.1岁,低血钾发生率为20%;12例由甲状腺机能亢进症伴周期性麻痹引起,平均年龄35.5岁,低血钾发生率为同期住院病人的8%;10例由糖尿病酮症酸中毒(DKA)引起,平均年龄21.2岁,低血钾发生率为同期住院病人的5%.治疗后,除少数病例低钾血症未能恢复至正常外(其中1例因治疗不及时而死亡),其余大多数病人均恢复正常。结论低钾血症在内分泌疾病中较常见,应引起临床医师的重视。
Objective To investigate the incidence of hypokalemia and its diagnosis and treatment in several endocrine diseases. Methods 58 cases of endocrine disease caused by hypokalemia were retrospectively analyzed. Results Fifty-nine of 15 patients were caused by primary aldosteronism with an average age of 41.0 years and a hypokalemia incidence of 100%. Fifteen patients were caused by Cushing’s syndrome with an average age of 36.8 years and low blood pressure Potassium incidence was 47%; 4 cases were caused by pheochromocytoma with an average age of 42.1 years and a hypokalemia incidence of 20%; 12 cases were caused by hyperthyroidism with periodic paralysis with an average age of 35.5 The annual incidence of hypokalemia was 8% of hospitalized patients in the same period. Ten cases were caused by diabetic ketoacidosis (DKA), with an average age of 21.2 years. The incidence of hypokalemia was 5% of hospitalized patients in the same period. After treatment, except for a few cases of hypokalemia failed to return to normal (including 1 died of treatment not timely), most of the other patients returned to normal. Conclusions Hypokalemia is more common in endocrine diseases and should be taken seriously by clinicians.
