目的通过案例分析,探索医疗纠纷产生的规律性因素,为医院的医疗安全管理提供客观依据。方法选取徐州市铜山区医疗纠纷案例30例,依据患方陈述材料、医患双方争议焦点及医疗事故技术鉴定书中专家意见,对纠纷的原因、争议焦点及专家分析意见进行归类。结果患者对治疗效果不满意是医患纠纷的主要诱因,占总数的53%,手术操作、治疗处理、告知沟通以及医疗文书方面的过失或缺陷是引起医患双方争议的主要原因。结论医疗纠纷发生的原因繁多,医疗事故的根源地是多方面的。医疗应加强医疗质量管理,进行充分而有效的医患沟通,对关键医疗行为建立高质量的医疗文件等,可以减少医疗纠纷及医疗事故的发生。
Objective To explore the regularity of medical disputes through case studies and provide objective evidences for the medical safety management of hospitals. Methods Thirty cases of medical disputes in Tongshan District of Xuzhou City were selected, and the causes of the disputes, the focus of the dispute and expert opinions were classified according to the patient’s statement materials, the focus of dispute between doctors and patients, and the experts’ opinions in the technical appraisal of medical malpractice. Results The dissatisfaction of the patients with the treatment effect was the main cause of the doctor-patient dispute, accounting for 53% of the total. The main reason that caused the controversy between doctors and patients was the negligence or defect in surgical operation, treatment, communication and medical documents. Conclusions There are many reasons for medical disputes, and the root causes of medical accidents are many. Medical treatment should be strengthened medical quality management, adequate and effective communication between doctors and patients, the establishment of high-quality medical documents for key medical practices, etc., to reduce medical disputes and medical malpractice.
