目的分析胰腺类癌的临床特征和CT表现,提高对该病的诊断水平。资料与方法3例经手术和病理证实的胰腺类癌患者,术前均接受了CT检查,其中1例接受了上消化道钡餐检查。结果3例患者均为胰腺单发的囊实性肿块,1例位于胰头、2例位于胰尾,肿块包膜完整,边界清晰。CT平扫实性成分的CT值为22～34 HU,平均27 HU。增强后实性部分在动脉期呈显著环形强化,CT值40～88 HU,平均55 HU,高于胰腺实质。门静脉期肿块实性成分强化程度明显降低,CT值50～72 HU,平均62 HU,略高于胰腺实质。其中1例患者接受上消化道钡餐检查,可见十二指肠圈明显受压移位,但肠黏膜未见破坏。结论胰腺类癌的影像表现具有一定特征性,结合病理检查,能为临床诊断提供帮助。
Objective To analyze the clinical features and CT findings of pancreatic carcinoid and to improve the diagnosis of the disease. Materials and Methods 3 cases of pancreatic carcinoid surgery confirmed by surgery and pathology were undergone CT examination before surgery, of which 1 case received upper gastrointestinal barium meal examination. Results All the 3 patients were cystic solid masses with single pancreas. One was located in the head of pancreas and the other two were located at the tail of the pancreas. The tumor mass was complete and the border was clear. CT scan solid component CT value of 22 ~ 34 HU, an average of 27 HU. After the enhancement, the solid part showed a significant annular enhancement in the arterial phase with a CT value of 40-88 HU, an average of 55 HU, which was higher than that of the pancreas. The enhancement degree of solid component of portal vein was significantly reduced, CT value of 50 ~ 72 HU, an average of 62 HU, slightly higher than the parenchyma of the pancreas. One case of patients underwent upper gastrointestinal barium meal examination showed that the duodenal ring was significantly pressure shift, but no damage to the intestinal mucosa. Conclusion The imaging manifestations of pancreatic carcinoid tumors have certain characteristics, combined with pathological examination, to provide help for clinical diagnosis.
