【目的】通过对比膝关节常规 MRI 序列,探讨三维扰相梯度回波序列(3D-FSPGR)对膝关节软骨损伤的诊断价值。【方法】对本院收治45例膝关节疼痛患者分别应用3.0T 3D-FSPGR 序列和常规2D 序列成像进行扫描。对不同序列图像分析,并以膝关节镜检查结果为“金标准”,比较3D-FSPGR 序列和常规2D 序列在诊断膝关节软骨损伤的敏感度、特异度和准确度。【结果】45例患者,共65侧膝关节,总共分析260个部分的软骨,关节镜结果显示0级的57个,1~4级的203个,3D-FSPGR 序列诊断的敏感性、特异性及准确性均显著高于2D 常规序列诊断,差异具有统计学意义(P <0.05)。【结论】3D-FSPGR 序列在诊断膝关节软骨损伤上要优于常规2D 序列,具有较高的临床应用价值。“,”Objective]To investigate diagnostic value of three-dimension Fast spoiled gradient sequence(3D-FSPGR)at evaluating cartilage injury of knee by comparing with the conventional MRI.[Methods]A total of 45 pa-tients with knee pain were treated in our hospital,who underwent 3D-FSPGR and the conventional 2D sequences at 3.0 T.Through analyzing these images,the sensitivity,specificity and accuracy were evaluated by comparing 3D-FSPGR with the conventional 2D sequence demonstrating degree of joint cartilage injury with the knee arthroscopy re-sults as the gold standard.[Results]Forty-five patients with 65 knees and 260 parts of the cartilage were totally ana-lyzed.The arthroscopy results showed that 57 parts as Grade 0 and 203 parts as Grade 1-4.The sensitivity,specificity and accuracy of 3D-FSPGR were higher than those of 2D sequences respectively,and the difference was statistically significant(P <0.05).[Conclusion]3D-FSPGR sequence is superior to the conventional 2D sequence in the diagnosis of knee cartilage injury,which has high clinical application value.
