正常窦性 P 波正常心电图中心动周期第一波叫 P 波,呈园丘状,顶端园钝光滑,是由窦房结发出冲动引起心房除极所产生的,故又称窦性 P 波。P 波正常极性是Ⅰ、Ⅱ、Ⅴs 导联一定正相,aVR 导联一定负相;时间0.06~0.11秒;振幅<2.5毫米。P 波的前半部约0.01秒,以右房除极为主;后半部约0.025秒,以左房除极为主;中间约0.04秒,系左右心房共同除极为主;两房激动结束,平均约0.075秒.由于两房除极存在先后,有些 P 波顶端显示两个小峰,表明两房激动的界限,此两峰间距正常<0.03秒.当房率增快时,两房除极的时间差值缩小,同步除极程度趋向一致,P 波顶端双峰可消失.一般 P 波振幅过低(<0.05毫米)或某些导联 P 波平坦甚至倒置,与 P 电轴有关,没有特殊临床意义.粘液性水肿,心包积液偶可出现 P 波低电压,此与电流传导
