目的了解脑卒中后抑郁患者的机体免疫功能状态。方法收集并分析南通大学附属南通第三医院2012年10月~2015年10月住院治疗的首次脑卒中患者90例[脑卒中后抑郁(PSD)、单纯脑卒中各45例],同时收集抑郁症、健康对照各30例,分析外周血T细胞亚群、免疫球蛋白及补体表达水平。结果无论PSD组还是抑郁症组T细胞亚群均不同程度地低于健康对照组(P<0.05),PSD组更为低下,与抑郁症组比较存在统计学差异(P<0.05)。无论PSD组还是抑郁症组IgG、IgA、IgM水平均较健康对照组下降,组内比较PSD组较抑郁症组更为低下(P<0.05),单纯脑卒中组和健康对照组IgG、IgA、IgM水平均处于正常范围,但单纯脑卒中组IGg水平较健康对照组低下(P<0.05);PSD组与抑郁症组补体C3水平均有不同程度地升高,PSD组较抑郁症组升高更为明显(P<0.05),而4组补体C4水平均处于正常水平,组间比较无统计学差异(P>0.05)。结论脑卒中后抑郁人群机体免疫功能存在不同程度的受损,可随病情持续进展,需加强动态监测,对于免疫力明显低下人群,可早期干预,阻止病情持续进展。
Objective To understand the immune status of post-stroke depression patients. Methods Ninety first-episode stroke patients (PSD, 45 stroke patients) were enrolled in this study from October 2012 to October 2015 in Nantong Third Hospital Affiliated to Nantong University. Meanwhile, depression , 30 healthy controls. The levels of T lymphocyte subsets, immunoglobulins and complement in peripheral blood were analyzed. Results The T cell subsets in PSD group and depression group were all lower than those in healthy control group (P <0.05), PSD group was lower, and there was statistical difference (P <0.05) compared with depression group. The levels of IgG, IgA and IgM in PSD group and depression group were significantly lower than those in healthy control group (P <0.05). PSD group was lower than that in depression group (P <0.05) IgM levels were in the normal range, but the level of IGg in stroke group was lower than that in healthy control group (P <0.05). The levels of complement C3 in PSD group and depression group were increased to some extent, and PSD group was higher than that in depression group (P <0.05). However, C4 levels in all four groups were all at normal levels, with no significant difference between the two groups (P> 0.05). Conclusions The immune function of post-stroke depressed people is impaired in different degrees and may progress with the progression of the disease. Dynamic monitoring should be strengthened. For those with low immunity, early intervention may be needed to stop the progression of the disease.
